Hello doctor,

I am a 52-year-old female. I am a non-smoker and non-drinker. I have a whitish-yellow spot on my right tonsil. On Saturday, I found this spot on my tonsil. I went to the doctor who thought it was a bacterial infection and put me on a course of antibiotics. I am into my third day, and still no change. It is scaring the crap out of me, to put it mildly. I have attached a photo. Not sure how long it has been there for which is also really worrisome. I have been known to have the odd sinus infection where my nose becomes blocked and clears. I feel the phlegm in the throat at different times. It has not affected my swallowing, and the doctor did not feel any enlarged lymph nodes.

Thanks for the briefing regarding the problem in concern. I had a detailed look at the picture you have sent us (attachment removed to protect the patient's identity). I am quite sure that there is absolutely nothing that you need to worry about. The white patch over the tonsil could be due to some underlying irritation over the mucosa due to allergy-induced secretions from your sinuses causing postnasal drip or could be due to silent acid reflux. Ulcers over the tonsils indicating a malignancy or cancer have a completely different look in their appearance and symptoms and are no way even close to what we can see in your case. There is absolutely nothing that you need to worry about. In most circumstances, these non-dangerous patches over the tonsillar mucosa persist for quite some time and have no or little response to antibiotics as they do not treat the patch directly. Anyhow, even if the patch persists, do not bother about it too much. You do not have to get it surgically removed either. Last but not least, refrain from searching the internet, as the internet has only one answer, and that is cancer. Please do not worry about this. I hope I have answered your query. Thank you so much doctor. Can you recommend anything such as gargling salt water that may help to get rid of this. I was afraid that it might also be some cyst. What you have told me has made me feel better. Do you think it could have something to do with my wisdom tooth as it is on the same side in which my wisdom tooth has not come through? Do you think it will ever disappear completely? Sorry for all the questions.

Good to hear from you again. Saltwater gargling can be considered. How effective it is to remove this patch is something that is questionable, and people have had variable results. It is improbable that the cause has been due to a wisdom tooth. It has a very low chance and the possibility of turning into a cyst, and even if it does, the patch has nothing to do with it in the first place. Yes, there have been patients whose patch over the oropharyngeal mucosa has reduced, disappeared and in some, it has persisted for years without creating any major trouble, and I have never seen patch turn into a malignancy unless it has been misdiagnosed in the first place or there have been certain risk factors involved unlike in your case. As mentioned earlier, there is absolutely nothing that you need to worry about. Even if it does persist for a long time, there is absolutely nothing that you need to do about it.
