Patient's Query

Hi doctor,

After taking Pentavalent vaccine my 2 month old baby is having loose motion for the last five days with tummy problems. Doctor suggests Ornidazole-Ofloxacin and Racecadotril suspension. Is it safe?

Answered by Dr. Deepak Patel

Hi,

Welcome to icliniq.com.

I see similar cases in my clinic every day.

1. Pentavalent injection (combination of five vaccines: diphtheria, tetanus, whooping cough, hepatitis B, Haemophilus influenzae type b) in a 2 month old baby does not lead to loose stools.

2. A baby can pass five to six stools in a day who is on breast milk. Since there is no fever, an infective cause seems unlikely.

3. In my opinion, there is no need of Ofloxacin-Ornidazole syrup and Racecadotril in baby.

4. If I was your treating doctor, I would have given Enterogermina (Bacillus clausii) ampule once a day for three days (prebiotic) to increase gut immunity and advised you to breastfeed the child regularly.

Patient's Query

Hi doctor,

I have given first dose at 2 PM first day and second dose at 6 AM the next day. My parents told me to discontinue the so I gave the third dose on second day 6 PM. My baby is passing yellow urine with smell. Along with the medicine I am giving Bismozyme drop and Pbpb sachet. I am afraid now as if it is an overdose and my baby stopped passing stool. Does this strong antibiotic cause any harm? Since prebiotic powder did not worked after two days I opted antibiotic but I am regretting now for any side effects.

Hi,

Welcome back to icliniq.com.

I have read your queries in detail.

I think that two or three doses will not do any harm to the baby, because in some infections like respiratory tract infection, pneumonia antibiotics are given to small babies. Antibiotics should be given only when there is a need and not unnecessarily. If given unnecessarily then only it is harmful. But again three to four doses will not be harmful. Secondly, you are saying that child is passing yellow urine with smell. This is because of prebiotic and the enzyme preparation you are giving. So, you should stop these medications as now they are also not required.

1. If child has more stools, then also the urine becomes concentrated and yellow so feeding should be done more regularly.

2. Stop all medications, feed child well.

3. There is no need to worry, in my opinion your child will be alright in 24 hours.

Patient's Query

Thank you doctor,

I am attaching the screenshot of the swelling area that my baby developed after taking Pentavalent vaccine before 15 days. Will it get cured automatically? Or is some medical treatment needed? He is not having pain in that leg. But over the swelling he has little pain I hope. We were applying ice before but now we have stopped. Also he has to take the second dose of vaccine in two weeks on the same leg. How can he take the vaccine if this has not fully cured?

Hi,

Welcome back to icliniq.com.

I have seen the picture (attachment removed to protect patient identity) you have sent.

1. The swelling is about 0.79 to 1.18 inches with redness over it. From the picture it looks like pus collection in the swelling.

2. In my opinion, it needs drainage immediately, because if drainage is not done then it may take long time about 10 to 12 days to heal with antibiotics.

3. You need to go to the pediatrician for physical evaluation of the injection abscess and drainage of the pus.

4. Do not massage over it and do not apply ice.

5. After drainage of pus antibiotic needs to be given like Cefixime for five days and local application of T-Bact (Mupirocin) ointment should be done for 7 days.

6. The due vaccine should be delayed if lesion has not healed till that time.

Same symptoms don't mean you have the same problem. Consult a doctor now!
