Hi doctor,

It has been 18 months since I have been suffering from allergic rhinitis. Some doctors are saying I do not have allergies. The problem is I keep sneezing even when I take Dymista. Now I am taking Allergodil nasal spray. I also got some blood tests done to detect if I have an allergy. I have attached my reports.

Kindly review my report, and help me know if I should be concerned by these results?

The attached report (attachments removed to protect the patient's identity) noted that monocyte and MPV (mean platelet volume) are slightly low. Do not worry about that non-specific finding. If they are elevated, then we can think further. But do not worry about its borderline low level. It does not indicate any leukemia or any blood disorder.

Your lymphocyte are slightly increased. It could be due to a temporary viral nose infection. So, it also does not indicate any blood cancer. However, after completing your medicine for allergy, I suggest you repeat your CBC (complete blood count) after one week. If lymphocytes are elevated even after one week, then we will investigate further. Do not worry. Mostly in one week, lymphocyte count will be normal.

Do not worry much about leukemia. Follow up with repeat CBC after one week. The high IgE (immunoglobulin) value in the attached report suggests that there could be some form of allergy as well in your body.

I saw your report (attachments removed to protect the patient’s identity). High MPV (mean platelet volume) has significance only when the platelet count is low. If the platelet count is low and MPV is very high, it could be due to underlying etiology like dengue, malaria, ITP (Idiopathic thrombocytopenic purpura), etc. But your platelet count is within the limit. So, do not worry about borderline high MPV.

However, your lymphocyte count is slightly high that is significant. We will follow up on that. After one week, repeat CBC (complete blood count), and if the lymphocytes are still high, we will think further.

Do not worry about cancer. It does not seem like blood cancer.

Thank you for the reply.

I received my CT scan results, and the doctor said I have hypersensitivity type-2 with a beginning of rhinitis but no infection that can cause high lymphocytes. So, he asked me to take some Cortisone. I have attached the medication he gave me.

Also, I got a new CBC. Can you please let me know if the results show more of a case about anemia and need some B12 and B6 supplements, or should I be concerned about eosinophil? Can it be cancer?

The eosinophil percentage is slightly high in the attached report (attachments removed to protect the patient's identity). So you can have type-1 hypersensitivity allergic reaction. Your MCV (mean corpuscular volume) is not much high, and hemoglobin is within range. So, the chances for anemia or vitamin-B12 deficiency are less.

I have already commented on lymphocyte count. It is slightly high. But does not indicate blood cancer. Repeat it after one week, and if the lymphocytes are high even after one week, we will discuss further.

You have been prescribed anti-allergic medicine, which can be continued.

If the lymphocytes remain high, investigate further with a peripheral blood smear to check for any abnormal lymphocytes. But mainly after a week, lymphocytes will become normal. It does not seem like blood cancer.

You have to tell the laboratory to do a peripheral blood smear examination report. It is done from 2 ml EDTA (ethylene diamine tetra-acetic acid) blood or finger-prick blood. Yes, wait for one week. Mostly it takes seven to ten days for the lymphocyte to settle down to the normal range in case of temporary viral infection. In the attached report, the lymphocyte percentage is borderline high and not elevated to the level we urgently need to investigate. So, do not worry.

The inflammation of liver is called hepatitis. Fatty liver also causes liver inflammation. Taking tablet Udiliv (Ursodeoxycholic acid) can be helpful. But before that kindly visit a nearby gastroenterologist for a physical examination and for the prompt management.

Kindly go through the attached blood test showing high SGPT, SGOT, and lymphocytes. My doctor told me that I have fatty liver. So I have a few questions. Can I have a normal cholesterol level when I have fatty liver? Do high SGPT and SGOT levels indicate cancer somewhere in the body? Is there any blood test to detect cancer? If yes, can you please tell me? I have high lymphocytes. Are they related to fatty liver, or can they indicate something else in the blood?

Excess use of multivitamins can harm the liver, so SGPT (serum glutamic pyruvic transaminase) remains elevated. It is advisable to stop multivitamins for a few days. You can take tablet Augmentin for the prescribed duration.

Yes, fatty change of the liver can cause an elevation in SGPT. You can make the following changes in your life to reduce SGPT.

1. Take a low-fat diet and less processed food.

2. Drink more fruit juice.

3. Take more lemon water and orange juice.

4. Citrus fruits can be taken more.

5. You can also include green leafy vegetables in your meal.

6. Do not take junk foods more.

7. You can start taking Ursodeoxycholic acid tablets for your liver problem.

8. Do mild to moderate exercise like brisk walking, running, or swimming for 30 minutes daily.

In some cases, fatty liver can present even with normal cholesterol. SGPT is specific to liver pathology. So, it indicates a problem in the bile canaliculi of the liver. And so, I suggested to do an USG (ultrasound) scan and visit a gastroenterologist. You can take tablet Ursodeoxycholic acid to slow down liver inflammation.

The attached report shows mild fatty change in the liver. For that, you can do lifestyle changes and take Ursodeoxycholic acid tablet. Lymphocyte elevation might be temporary due to viral infection. Do not be worried. It does not seem to be cancer. If lymphocyte becomes elevated again after few days, repeat CBC and investigate further with peripheral blood smear examination report to look for any abnormal lymphocytes.

A peripheral blood smear is done from finger-prick blood or EDTA blood. It is a simple test in which the RBC (red blood cells), WBC, and platelet studies are done in detail, which is not possible in CBC. Lymphocyte counts are not related to liver health. Usually, it remains elevated in viral infections. Yes, in the attached report, absolute lymphocyte count is normal. There is a slight elevation of eosinophil, which might be caused by allergy inflammation. Do not worry. Repeat your CBC after a few days. For high SGPT, follow the lifestyle change mentioned above.

My further opinion for your questions are as follows;

1) You can repeat CBC with peripheral blood smear after ten days.EDTA blood is needed for that.

2) CBC does not study the cell morphology and manual differential count.

3) In peripheral blood smear, the pathologist will prepare the slide and will see cell morphology especially white blood cells morphology in detail. So, I recommend doing that test also.

I have seen your reports. Your peripheral smear report do not show any abnormalities in WBC. In case of leukemia, immature cells will be seen. Here immature cells absent. So it does not seem like leukemia. Macrocytic RBC means increase in RBC size which is a benign condition and commonly seen in B12 deficiency.
