Hello doctor,

I am suffering from a genital skin problem. There is a red color spot with inflammation on my glans penis. I had used antihistamines, antibiotics, and antifungals but they were just temporary relief for the last five months. I am a 29-year-old male. Currently, I take a capsule of Cefixime 400 mg once daily and a tablet of Cetirizine 5 mg twice daily. Please help.

With your descriptions and the image provided (attachment removed to protect the patient's identity), it seems you have developed balanitis.

Balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene and overwashing, use of over-the-counter medications, and non-retraction of the foreskin. It may be associated with diabetes and candidal fungal infections.

You should go for the following investigations- blood sugar and KOH mount test for candidiasis.

Treatment-

1. Gentle retraction of the foreskin daily and soak in warm water to clean the penis and foreskin.

2. You can apply a combination of Fusidic acid cream and Clotrimazole twice daily for one month. If balanitis does not respond to topical therapy, then you can take a tablet of Fluconazole 150 mg once a week for four weeks. In recurrent cases, 1 % Pimecrolimus cream should be used.
