Hello doctor,
I am 39 years old. I feel heart pain at midnight or in the early morning when I wake up. This pain releases when I pass urine. For urine retention and enlarged bladder (my USG detected 500 ml), I am under medication, Silidosin, and Urotone. In the morning, sometimes I feel like ejaculating, but I am neither aroused nor clear urge. I am confused about whether it is urine or sexual urge, but I wake up thirst and heart pain.
I have a history of GERD and varicocele surgery. My recent PET scan is normal.

Yes, my Vitamin D is low, and even my sugar tends to be low many times. I feel sudden hunger and yesterday fasting was 66 only. No, I do not ejaculate in the sleep in the morning.
Yes, I have itching and allergy beneath the anus (recently) and on both the buttocks, since few months. And I do not have nightfall.
