Hello doctor,

I have this blood blister since one month which I am very concern about it. Its soft but hard to break it and I broke it once which lots blood came out. And I also have open pores, pigmentation, pimple spots. And dry skin but its pimple prone, and also I have lots of pimple spots on my back and some pimples too which I am using Salyzap spray, I also use vitamin C serum plus Kojivit ultra at night. My main concern is about the bloody kind blister on my forehead.

Blood blisters occurs when tissue and blood vessels are damaged due to friction or trauma and is more painful than regular blisters. It usually resolves on its own within one to two weeks. You have not mentioned any history of trauma or friction before onset, also it is present for one month and not going away. After looking at the picture (attachment removed to protect the patient's identity), it looks like a condition called pyogenic granuloma, which is a common benign growth that appears as rapidly growing, bleeding bump on the skin, composed of blood vessels. Treatment is done by excision, electrocautery, cryotherapy by a dermatologist. You should not try to peel off the skin or damage it by your hands as you can get secondary infection. For your open pores, there are some ways to reduce their size by using a clay mask, using noncomedogenic water based products that would not clog pores, washing face twice daily with facewash containing BHA (beta hydroxy acid) like salicylic acid 2 percent, apply a toner before applying oil free moisturizer, wearing sunscreen of SPF (sun protection factor) 30 or above daily. Drinking plenty of water. There are some cosmetic procedures that can be done by a skin specialist like microneedling, chemical peeling. For black spots you can continue using vitamin C serum. Kojic acid or azelaic acid or arbutin or mulberry extract containing cream can also help in removing spots.

This blood blister came on its own without any trauma or friction, but it appeared after I started applying castor and coconut pure oil every day. It does not cause any problems since it is painless and tender, but I am concerned about basal cell carcinoma and melanoma.

You cannot get a blood blister from applying castor oil and coconut oil. Basal cell carcinoma is a slowly growing tumor that develops over months and years, and it is locally invasive. In melanoma, there is a positive family history, and the skin lesion appears dark; it can also occur in a preexisting mole if a mole starts to change color. If you are concerned about these disorders, there is a very simple and non-invasive technique called dermoscopy, which can be easily performed by a dermatologist. And you can receive the test results very soon.

According to me, it has a short duration of one month, so the chances of malignancy are not very high. But you can consider dermoscopy and excision or electrosurgery.

  1. For open pores, use any face serum containing retinol 2.5 percent, vitamin E and hyaluronic acid. Apply it at night after washing your face. Use toner (like rice water moisturizing toner) after washing your face and before applying moisturizer during the day.
  2. For acne spots, you can use Aziderm (Azelaic acid 15 percent) on dark spots at night. During the day, apply sunscreen with SPF 30 or higher. If you cannot find Aziderm 15 percent, you can use Azobril 15 percent cream. Both have the same composition.
  3. Regarding the bump on your forehead, you can wait and watch for another month. Do not touch, manipulate, or peel the skin. Apply Mtaz (Mometasone) cream twice daily over the bump for two weeks. If it still does not reduce in size by then, you can consider removal.

Yes, you can use Momate cream; buy a 5 gram pack because you need to apply only a small amount over the bump twice daily. Use it only for two weeks and then stop. Do not apply Mometasone over normal skin.

The bump seems to be improving by applying Momate cream, but I had already broken it before using the cream. I am using vitamin C serum and Kojivit ultra cream (a combination of octinoxate, niacinamide, glycolic acid, kojic acid, arbutin, vitamin E, and others) at night, so can you please tell me which one should I apply first, Kojivit Ultra or vitamin C serum? Also, I have big and stubborn stretch marks on my waist and hips. Can you please recommend a cream or any treatment to lighten those stretch marks? One more thing, there are many pimple spots on my back. Please recommend a cream to get rid of those spots. Please note that there are many spots, so recommend a strong cream that can remove them easily.

Good to know that the bump is improving. First, after washing your face, apply vitamin C serum to your face and let it absorb into your skin. After five minutes, apply Kojivit ultra cream only to the areas with hyperpigmentation, using small amounts. You can apply it using earbuds or a cotton-tipped applicator. For stubborn stretch marks, use Stretchrid cream twice daily and massage it into the marks for 30 seconds. For pimple spots, use Demelan lite twice daily.
