Hi doctor,

My mother is having leg cramps from the past two to three months. And now from the past two weeks, she is complaining about feet sole pain and burning sensation which is almost intolerable. We got a few blood tests done as advised by our family doctor as per which two tests are worrying us. Glucose fasting and plasma reading came out to be 129 mg/dL and Vitamin D 25-Hydroxy serum reading was 30.22 nmol/L. So, kindly advise how to proceed further.

I went through the history and mentioned investigations (attachment removed to protect patient identity).

Fasting blood sugar 129 mg/dL is something to worry about because it is in the diabetic range. I will be happy to help you better if you can upload all the investigation results for my reference. Her sugar at present in the diabetic range newly diagnosed, her burning sensation and sole pain could be due to diabetic neuropathy.

We need to check the post-meal blood sugars also and start her on antidiabetic medicines. Until then take the medicines as advised below.

Investigations to be done

HbA1C (glycated hemoglobin), PPBS (post-prandial blood sugar), Urine routine.

Probable diagnosis

DM. Diabetic neuropathy.

Treatment plan

Tablet Pregabalin 75 mg twice daily for two weeks, Capsule Renerve plus after dinner for one month.

Preventive measures

Take plenty of water. Avoid sweets. Reduce oil.

Regarding follow up

Upload the rest investigations reports Also, do the other investigations as advised.
