Hello doctor,

I have burning sensation in my penis since last month. I went through all tests like blood tests, urine tests, CAT (computerized axial tomography) scans, and cytology. Everything is normal except for the microscopic hematuria finding in the urine sample. The urine culture test is negative and PSA (prostate-specific antigen) test is normal. When my bladder is full I get a signal by pain in my penis to empty it and feel relaxed after emptying it. But after that, I get a burning sensation that lasts for at least half an hour.

Coming to your issue, could you please help me with a few details: Are you diabetic? Are you a smoker? If yes, how long have you smoked? Have you ever had any visible blood in your urine? Do you have any issues while passing urine like - straining to pee, increased frequency, urgency, or feeling of incomplete bladder emptying? Do you have to wake up multiple times at night to pee? Are you sexually active? If so, is there any pain after ejaculation? Is there any pain in the area between your testicles and anus? Do you hold urine for a long time before passing it?

Thank you for your reply.

I am a nonsmoker, do not have diabetes, no visible blood in urine, and no problems with passing urine. I woke up two times at night time and it started one month back with frequent urination and urgency to urinate. But now it has decreased. I have pain in my anus (same burning sensation as in my penis), no problem with ejaculation, pain during intercourse, and not being able to hold urine for a long time.

I understand your concern.

I could only see the cytology report which is negative for any high-grade cancer and that is good news. Unfortunately, that does not rule out the chance of any low-grade cancer in the bladder. Did your healthcare provider order an ultrasound or contrast CT (computerized tomography) scan for you? In case you have trouble uploading the files, could you just manually type in the results? Did they offer you a cystoscopy to check your bladder from the inside? What is more feasible at your age is that you may have some degree of benign prostatic enlargement or chronic prostatitis. Both of these conditions can cause microscopic hematuria due to your urine rubbing and irritating your enlarged prostate. Both of these can be managed successfully with a medication called alpha blockers along with probiotics and a urinary cleanser containing hibiscus extract and D-mannose. They help improve your urinary stream and reduce symptoms.
