Hello doctor,

I have itching on my foreskin. I observed creamy deposit behind the foreskin reappearing every day despite washing my penis by pulling foreskin. I feel very itchy on the penis when I pull back the foreskin for washing. Now I am using Cetaphil. This problem has occurred three times in last six months. Cetaphil has helped. I am worried about frequent reccurrance. Should I meet a dermatologist?

Thank you doctor,

Images are attached. The deposits are approximately 24 hours after the bath and cleaning my penis. Earlier there was some redness, but now I think it is gone and itching has reduced and the feeling to rub the foreskin. I have had both unprotected and protected sexual intercourse with my wife and been careful by using protection in case of sexual intercourse with any other woman (very rare, maybe only once in past one year).

You have a white collection of matter around glans which is a sign of fungal infection (attachment removed to protect patient identity). This is candidal balanitis. If you are not allergic to oral antifungal medications then consider:

  1. Oral capsule Itraconazole 100 mg twice daily after meal for seven days.
  2. Apply Clotrimazole 1 % twice daily (DK gel).
  3. Wash area with lukewarm water.
  4. If your partner has a similar complaint then treatment for her is also required.
  5. Get a blood test for fasting blood sugar level, postprandial blood sugar level and revert back with reports.
  6. Wash clothes separately in hot water.

Thanks for your reply.

I will follow your recommendations.

My wife has not complained. What type of symptoms should I expect? However, I have observed occasional strong odor from her vagina. I got my blood tests done a few days back. HbA1c and fasting sugar were included in the tests and the report is attached for your reference.

The lipid profile is slightly raised (attachment removed to protect patient identity). Avoid fatty food. Blood sugar levels are normal. If your wife has a history of white discharge per vagina or itching then she should be examined before giving medications. Avoid sexual contact till your symptoms subside.
