Hello doctor,

I am 38 years old. My first IUI failed. Then one month gap. Now, today is the second day of my period. He told a retained follicle is present after scanning and prescribed Dronis 30 daily. When I get my period again, he told to come on the second day of the period for a scan and start my second IUI. Is Dronis useful because it is a contraceptive? I am trying to conceive naturally also in this gap. What should I do? Please guide me.

Dronis (Ethinylestradiol) is a contraceptive pill also used to reduce the size of the ovarian cyst. You will not conceive in the month you are taking the pills. But, it is necessary for the cyst to resolve before you can be given medicine for ovulation induction. So you should take Dronis. You may have to take it from one to three months depending on the size of the cyst. If your cyst is absent next cycle, you can be given medicine for ovulation to happen. It would be great if you could lose weight, at least 6 to 8 lbs. As you are 38, do AMH (anti-Mullerian hormone) blood test and antral follicle count on the second-day scan to know your ovarian reserve. If it is low, I suggest you go for IVF since you are elderly. Do not delay much. Are your TSH (thyroid-stimulating hormone) and prolactin normal? If you can send me your reports, I can guide you better. Investigations to be done include:

  1. Serum TSH.
  2. Serum prolactin.
  3. AMH.
  4. CBS (complete blood count).
  5. PPBS (post-prandial blood sugar).
  6. Baseline scan on the second day with antral follicle count.
  7. Husband's semen analysis with three days of abstinence.

I suggest taking tablet Ovacare (Multivitamin and Multimineral) daily one till pregnancy and tablet Folic acid 5 mg daily once.
