Patient's Query

Hello doctor,

My husband has acquired external hemorrhoid, which is small and bleeding.

Hello,

Welcome to icliniq.com.

I have read your query.

Does your husband have chronic constipation? Is there any mass or lump coming out while defecating? There must be pain present along with bleeding. Is your husband taking any medication? Is it possible for you to send a perianal picture to assess the external swelling? Ideally, you should visit a local doctor to confirm external hemorrhoids.

Patient's Query

Thank you doctor for the reply,

He is not taking any medication. He has been applying RectiCare advanced hemorrhoidal cream. It is only bleeding from a little blood blister that has formed on the top. He has no pain.

Hello,

Welcome back to icliniq.com.

You have mentioned there is no pain at all. You did not reply whether there is constipation present or not. Symptom of pain is one of the distinguishing features between external and internal hemorrhoids. If there is painless bleeding from the rectum, then it is internal hemorrhoid. If bleeding from the rectum is associated with pain, then it is an external hemorrhoid. From the attached images (attachments removed to protect the patient's identity), I can see there is swelling at the anal opening with dark colored blister over the swelling. It appears to be an external hemorrhoid (but there is no pain during defecation). RectiCare advanced hemorrhoidal cream contains Lidocaine and Phenylephrine as active ingredient, and I, too, would have suggested cream with the above composition to apply over the anal opening, but you are already applying it. The home remedies of hemorrhoids include:

  1. Eat high-fiber foods. It will soften the stool and increases its bulk, which will help you avoid the straining that can worsen symptoms from existing hemorrhoids. Drink plenty of water every day.
  2. Take stool softening medication if there is constipation. Avoid straining while defecating, as it would aggravate swelling and bleeding.
  3. Do sitz bath. Soak your anal area in plain warm water for 10 to 15 minutes two to three times a day.
  4. If the above symptom has been present for about a week with no improvement after the application of hemorrhoidal cream, then you should definitely visit your doctor regarding it.

These are usual treatment suggestions for external hemorrhoids. If you find any relief with the above measures, then it is very good, and if there is no symptomatic relief, do not wait and watch. Kindly visit your doctor, who might consider or recommend a surgical procedure.

Patient's Query

Thank you doctor for the reply,

He does not have any constipation or pain.

Hello,

Welcome back to icliniq.com.

Initially, he would have felt it while developing a dark blister, and now blood is oozing from it. He needs antibiotics and a sitz bath regularly to heal the lesion. I would suggest Oflox Oz (Ofloxacin and Ornidazole) antibiotic two times a day for five days (I hope he is not allergic to the antibiotic), but I know you need a prescription to buy an antibiotic. Please visit your doctor as he needs antibiotics and a sitz bath regularly. He would not need surgery. Do not worry about it. Most likely blister would fall off as it appears to be necrosed but visit your doctor as early as possible.

Same symptoms don't mean you have the same problem. Consult a doctor now!
