Hi doctor,

I sometimes suffer from abnormally frequent urination, up to ten times in one hour, but I find all my clothes smell of urine. Although I am entirely sure of complete emptying, it is clear that there are drips and a foul odor, even with a good cleaning, especially after urinating or during and after sex. I needed advice about a problem and tried to treat it, but there were no results. Kindly let me know if you need laboratory tests. Please help me.

I understand your concern.

As I can understand, you are having long-term problems with severe urgency of urination. You have not mentioned the frequency of urination at night and whether you have done any sonography or CT (computed tomography) scan of the abdomen. You are already taking medication for the frequency of urination, which you have mentioned. So even if the medication problem is not being solved, further tests are required to find the cause of the problem. Firstly, you need to do the sonography of the abdomen and pelvis and then test to rule out other conditions like prostatitis and tuberculosis of the bladder, which are one of the most underdiagnosed yet common causes of severe urgency and frequency of urination. Next, you will need a test called urodynamic study. Urologists do this test in their clinic, and it precisely measures the pressure inside the bladder and thus helps to find the cause of the urine frequency. I suggest you continue Betmiga (Mirabegron) 50 mg twice daily if your blood pressure is normal for symptomatic relief. Once the test, as mentioned earlier, finds the exact cause of the urine frequency, then specific treatment can be planned. Premature ejaculation is a different problem and is usually unrelated to the cause leading to urine frequency. Simple measures like using a condom, using Lignocaine jelly over the tip of the penis, and the first before starting any medication.

I understand your concern.

I went through your reports (the attachments removed to protect the patient's identity) and it suggest mild urine infection, but since you have already taken antibiotics, that is not a significant concern. If your sonography report is normal, further investigation is required to find the cause of your problem. According to the reports, you must contact a urologist for tuberculosis screening and urodynamic study and plan further treatment.
