I have pain in testicles with swollen glans and abdominal pain. Why?

I have a symptom similar to gonorrhea and chlamydia with chronic testicles pain and swelling at the tip. Glans has a weird color, and pain under the abdomen sometimes. The urine test reveasl negative for STD. I am waiting to see a urologist and it has been for a year, now symptoms seem to change and getting worse and I am getting worried.

From history, it seems you have pain in testicle and at the tip of glans. I need to know about frequency of urine at night, urgency to pass urine, sense of incomplete voiding? How is the flow of urine? Any straining to pass urine? Unprotected sexual exposure? Constipation? How is the pain in testes aggravated? Any associated swelling or redness of scrotum?
Kindly share a picture of the glans abnormality you describe.

Frequency of urine is fine at night, but the bladder hurts sometimes when I wake up in the morning. No problem with the flow and urgency depends on what I drink. For example, coffee it is like 20 times a day if not more. Drinking a lot of water really helps with the symptoms. I had unprotected sex in the past before having these symptoms and the test reveals negative. I have sometimes pain under the abdomen and gives an urge to poop diarrhea but no constipation. Testicle pain seems to appear after masturbation and gets better with time or the pain appears when I am driving moving around or different body position helps. The pain is like a pinch. I also have this pain between the testicles and the rectum like muscular pain usually shows up after I masturbate. So for the swelling, the only place I notice is just the gland and the color gets marble between white and red when I masturbate it is hard to explain. Scrotum seems fine to me. I will send a picture of it.

Perineal pain, testicular pain, urgency to pass stool with redness at the tip of glans is your concern. Testicular pain after masturbation is common in young males due to cremaster contraction which suspends testes while it passes from intraabdominal location before birth to the scrotal position finally. Using scrotal support or langot (cotton strip used by wrestlers to keep testes close to the body and not hanging) during day time will help by not allowing the testes to hang thereby relaxing the cremaster muscle used to pull them up. Perineal pain and urge to pass stool is a part of chronic pelvic pain syndrome. You need to perform perineal exercise (intermittent tonic, clonic contraction of perianal muscles 15 minutes twice daily) or yoga. Take psyllium husk three tablespoons in one glass water at bedtime. The redness at the tip of glans if no discharge per urethra, no burning urination, good flow then can be observed for any progression.
