Hello doctor,

I am having left-sided headache, mostly frontal and behind my eye for a week. I have ear pressure and pressure in my nose while moving and chewing. I have seen my primary care physician (PCP), and I was given Imitrex that does not help. I have tried Aleve, Motrin, and Excedrin.

Welcome back to ilciniq.com. I do not think that the drugs are helping because if this is a sinus infection, antibiotics are necessary and painkillers do not help much. Do you have a running nose or blocked nose? This would be another argument in favor of a sinus infection. For this reason, it is necessary for performing a sinus X-ray study and some tests like complete blood count, PCR (polymerase chain reaction), ESR (erythrocyte sedimentation rate). Yes, I think that you should consult your general physician and ask for the above tests to investigate for other underlying secondary causes for headaches (sinus infection, inflammation or anemia). I do not think this is migraine because it is lasting so long. Have you ever experienced this type of headache? Is it like pressure or like throbbing? Do you have any nausea? Is the headache worsened by any body position and is it worse in the morning or the evening? Do you have any allergies? Do you have a blurring vision in the left eye? It will help if you discuss with your general physician on the above tests. Let me know in case of any other uncertainties.

I have a throbbing-type of pain. Yes, allergies occur while taking Zyrtec and Singulair. Nausea may be due to medicines, worsens while chewing, and there are no visual changes.

The fact that chewing is triggering the headache could indicate a possible temporomandibular joint inflammation or atypical headache. Anyway, because the headache is lasting for such a long time, I would recommend consulting with your general physician and performing the above tests. A brain Magnetic resonance imaging (MRI) would be the next step in case of normal sinus X-ray study and blood lab tests and if the headache persists.
