Hello doctor,
I am a 51-year-old male. Whenever I have sexual intercourse with my wife, I get a sore throat and a flu-like condition. Whenever I have anything cold, my throat gets infected. I also have diarrhea. My wife is also suffering from the same problem, and my son has diarrhea sometimes. All this has been happening for the past three years. I had sexual intercourse with my girlfriend 10 years back, so last year I did a rapid and tri-dot test for HIV, which was nonreactive. Do I have HIV? Kindly guide.

It does not appear to be HIV (human immunodeficiency virus). The symptoms of HIV will be entirely different, and you also tested negative. But if you still have doubts, then you can retest.
Getting a sore throat after sex is probably a coincidence, and a sore throat after taking cold items is possible. It is due to the multiplication of the bacteria. To avoid this, try to stop taking cold items, and even if you do consume them, then eat them gradually and avoid direct contact of cold items with the throat. Let it get a little warm in the mouth before swallowing it. You can also try betadine gargles, three to four times a day, once you get the feeling of a sore throat. It will prevent the multiplication of bacteria or viruses.
Diarrhea is mostly due to indigestion. Do you consume a lot of milk products, chickpeas, etc.? Do you also have upper abdominal pain, bloating, burping, nausea, vomiting, etc? I suggest you try doing the following-

Thanks for your reply. Why is my wife having the same symptoms? She is not eating properly due to the fear of having diarrhea. I am worried because she is also losing weight without doing any exercise. Is the HIV test conclusive?

HIV tests have a very high sensitivity. It is highly unlikely that you have HIV. Diarrhea will not respond to fasting; it will continue regardless of whether one has food or not. Also, it is unlikely that HIV patients in a family have only diarrhea and no other symptoms. Patients with HIV will be very sick, and diarrhea will occur 10 to 15 times, loose, significant weight loss, say 11 to 22 pounds in a month, fever and many other symptoms. Digestive problems are very common, and it is possible that two people in the same family have them, especially worm infestation. How many times do you people need to use the toilet? Is it a loose or well-formed stool? Is there any abdominal pain? Any factors that increase or decrease the stools? Although it will be difficult, try to get your wife tested for CBC (complete blood count), HIV, liver and kidney function test, stool routine test and thyroid test.

Thank you for your reply. To clarify, my wife and I do not always have loose motion. But we need to go at least two times. For me, once a week, my stools are not normal. Secondly, I have had throat infections on and off several times. Secondly, as soon as we both get up, the first thing is that we have to go to the washroom. Sometimes, my wife feels very cold in the night. In your opinion, it seems that we both do not have HIV. I just wanted to know whether the test that I have undergone is conclusive.

So, I do not think it is diarrhea. It is just a matter of bowel habits and a change in stool due to food. It is indeed not related to HIV. For feeling cold in the night, just get one thyroid test done. From the symptoms, it is not HIV. As both tests are negative, the chance of HIV is almost nil. These tests are highly sensitive.

One more symptom that I forgot to tell you is that I have developed an ulcer inside my penis. Hope nothing related to HIV, but I had read one of the signs is an ulcer.

No ulcer, per se, is not caused by HIV but by other sexually transmitted diseases, which often accompany HIV. Where exactly is the ulcer on the shaft or glans penis? Is it painful or painless, single or multiple ulcers? Was there any vesicle before the ulcer? Does your wife have a similar ulcer now or in the past, or any white discharge? I guess there has been no exposure to outside sex in recent times. The various possibilities are syphilis, herpes or a few other infections. You should get your VDRL test for syphilis and serology for HSV2. I will suggest the treatment after tests, or at least with the details of complaints, so that I can have an idea about the diagnosis.
