Hello doctor,

I am experiencing unsettling thoughts about reality, where I question everything, and despite not leading to anxiety or panic attacks, they still make me feel scared. These thoughts persist consistently. I find myself pondering the possibility of nobody around me being real and the notion of living in a dream. Additionally, there is a concern that there might be something happening around me that I have yet to comprehend. These thoughts evoke a great deal of fear, as I am genuinely afraid of fully embracing them and potentially becoming psychotic. They significantly impede my ability to function normally, placing an overwhelming amount of pressure on me. Even when I manage to convince myself that these thoughts are irrational, there are instances where I sink into a profound depression that becomes too much to bear. Currently, I am not taking any medication. Despite having periods or days when I feel completely fine, these thoughts persistently resurface and refuse to dissipate, no matter how hard I attempt to dismiss them through logical reasoning. These thoughts are particularly distressing because I am unable to provide answers for all of them, especially those that delve into existential aspects that science is presently unable to fully explain. The fear of losing my sanity and descending into psychosis is terrifying to me.
