Hello doctor,

I have itching and burning penis with a red rash which burns sometimes and it has been there for 20 days. It reduces and increases some times. I have used FourDerm and Candid B but what could be the problem? Kindly suggest.

It seems you might have a fungal infection and the creams you are using are harmful in that. So stop using them at once. Send me a picture of the area, if possible. Meanwhile, apply the cream I am prescribing below:

1. Tablet Sebifin 250 (Terbinafine) once a day.

2. Tablet Levosiz (Levocetirizine and Montelukast) 5 mg at night.

3. Cream Lulifin (Luliconazole) apply at night.

It does not look like a fungal infection. As you sent me the picture, I have seen it (attachment removed to protect patient identity). You have superficial erosions and ulcers. It can be herpes genitalis by the look of it. I have a few questions for you to understand your condition better.

  1. Did the redness start as fluid-filled blisters?
  2. Do you have any history of sexual exposure in the last year or before?
  3. How painful are they?
  4. Anywhere else on the body do you have this problem?

Kindly ignore the medicines in the last message and follow these for five days, and it will be better before that if you answer these questions.

1. Tablet Acyclovir 400 mg three times a day. 2. Cream Fucibet (Betamethasone and Fusidic acid) twice a day for five days.

  1. No, it started with itching and redness.
  2. Yes, I am married, and I have sexual exposure.
  3. There is no pain. Only after intercourse, there will be a burning sensation.
  4. It is nowhere else except there.

I started using the cream earlier you advised for last night. Is that fine and can I start with the new one from today?

Thank you for the reply.

There has been no burning sensation or any other issue after using cream and tablets. But, there are white patches around the area. Please check the attached picture for reference. It was taken just after the bath.

It seems that cream and secretions have piled up there. You need to remove the whitish material with wet cotton and normal soap gently. Clean it with soap and water first and gently try to scrap it off with wet cotton. Please do not force it. Just apply any moisturizer after removing it or use Nevlon cream twice a day and send a picture after removing it.
