Patient's Query

Hi doctor,

I am 30 years old. For the past few years, I have been suffering from itching in my groin and surrounding areas. I usually use Cloritamazole dusting powder on and off. But, when the itch really increases, I use Quadriderm ointment for 1-2 days. Once the problem reduces, I stop using it. For one year, I have used one tube of ointment and about 2-3 boxes of powder. I have never consulted a doctor regarding this. But, now the problem has increased and I am looking for the right treatment. Since, six months, the itching has become severe on the groin, scrotum, and penis. I can notice, there is a lot of flaking and chafing of the skin around it. Sometimes, I can find blood spots in that region. I have an itching sensation even while sleeping. Generally, I wear loose clothing or no undergarments and spend most of the time in the comfort of the AC. I have no other skin problems. For a month, I am using pears soap and washing the area with lukewarm water twice a day. Occasionally I take the tablet Allegra for itching. For the last fifteen days, I am using Clocip dusting powder continuously. I am under homeopathic medicine for chronic sinusitis. But, need to take antibiotics once every 3-4 months. Four years back, I was diagnosed with internal hemorrhoids. As of now, I have no problem associated with it. I have attached the recent pictures. Please check and suggest a permanent treatment for the same and also let me know what precautions I need to follow. Thank you in advance.

I can very well understand what you are going through. I will give my best shot to treat your problem. I examined the photographs (attachment removed to protect patient identity) and found the original fungal lesions have been converted to the present appearance because of medicine. It is a common tendency for a person to use local steroids like Quadriderm ointment to get relief quickly. This is when the problem gets complicated. My advice would be to never use such products. But if already used no need to worry. Once a physical examination is done we can find a good solution.

My suggestions for you to follow are:

  1. Maintain the area dry and use undergarments only when the area is completely free from moisture.
  2. Wear loose cotton undergarments that will absorb all the moisture content.
  3. Cut down on intake of spices, caffeinated drinks, fried foods, and alcohol.
  4. Stop using Clocip dusting powder immediately.
  5. I would suggest you to start Eumosone-M cream (Clobetasol propionate) twice a day for a period of seven days. If applying twice is not working and itching does not get better, then you can use it for another one or two time a day.
  6. Most of the patients have found the tablet Levocetirizine (Levocetirizine hydrochloride) 5-10 mg beneficial. But, the main draw back of this tablet is, it tends to get the patient drowsy. Tablet Allegra (Fexofenadine) too can be taken.
  7. For fungal infection you can take tablet Terbinafine (Lamisil) 250 mg twice a day.

For the last four days, I have been taking the tablets you prescribed as well as applying the ointment twice daily. I can feel a lot of improvement. I have a few questions. I would like to know, can I use pears soap itself to wash the area or change the soap? I wash the area twice with pears soap before applying the ointment. For how many days should I be taking tablet Terbinafine 250 mg and at what duration? Can you let me know, if these medicines need to be used always or is there a permanent solution for the same? I have attached the new pictures, please do have a look and guide me accordingly. Thank you.

I have checked your new photographs (attachment removed to protect patient identity) and details. It is heartening to note the tremendous change. Below are the answers to your queries.

  1. Soap can be used before applying the cream. Choice of soap does not actually matter much.
  2. Initially, tablet Terbinafine is meant to be used for two weeks. Further use depends on the condition at the end of this period.
  3. The white specks observed in the photographs are mostly a sign that the lesion is healing.
  4. The problem can be controlled by reducing sweat, friction, and controlling the precipitating factors like obesity, diabetes, and activities like running.
  5. Lifelong treatment is not required. In case you need medicine for an intermittent period, then you can use topical application for a short duration.

Same symptoms don't mean you have the same problem. Consult a doctor now!
