Hello doctor,

I have three daughters and want to do IVF (gender selection for the next baby). But I am a poor responder and my age is 34. This cycle, I took Gonal-F 300 but had only three follicles; my antral count was eight in total. What is the option in my case? Please help.

I understand your concern.

Being a poor responder only means that you give a lesser number of eggs. But your age is not so high, so whatever few eggs you get will be healthy enough to give healthy embryos and healthy babies. Having a male baby mainly depends on the sperm, not the egg. In ICSI (intracytoplasmic sperm injection), the fastest-moving sperms will be selected. Usually, the Y chromosome carrying sperms are lighter, more sensitive, and thereby move faster. So more chances to have a male baby.

Correct any health issues like diabetes, infection in urine or semen, smoking or tobacco use, etc., in a husband to improve the chances. This would increase the Y chromosome carrying sensitive sperms, which can get destroyed by these factors. To improve the chance, have a PIGD (postural instability and gait disorders) and determine the embryo's genotype.

I am sure you will have what you want. If this still fails, have a break of three months and try another cycle with the long protocol. This is better for a lady with a low response. Before that stimulation, even treatment with PRP (platelet-rich plasma) can be tried; in some, there may be improved follicular response.
