Hello doctor,

I was seen in the ER for a possible kidney stone. I did have blood in my urine. No stone was found. The only thing noted on my CT scan reads “circumferential urinary bladder wall thickening versus under distension”. What does this mean? Is this a sign of bladder cancer? What follow-up is needed?

I need to know some more details of the hematuria episode. Was bleeding associated with pain? What was the location of pain? Was it radiating to the back or testes or penis? Were there clots, shape, and size, if present? How frequently do you get up at night after going to bed? Do you need to rush to pass urine? How is the flow of urine, do you need to strain to pass urine? Any dysuria? Do you have a sense of incomplete voiding after passing urine? Any family history? Do you have any reports like urine analysis done at that time?

Thank you doctor,

I had back pain. That was the reason for the ER visit. No clots, it was microscopic. Report said RBC 3 to 10. I do not feel an urgent need to urinate, nor do I get up at night. No real pain with urination. Every now and again it seems like the back of my testicle burns. Nothing major, just very focused on any sensation now.

Back pain with microscopic hematuria is very likely due to a stone passed (common presentation). No significant lower urinary tract symptoms. CT is more likely representing under-distension, unlikely any cancer. Repeat urine analysis after two weeks. If no recurrent hematuria, no further evaluation (repeat urine R/M). If recurrent microscopic hematuria is present, you need to get cystoscopy done and a contrast-enhanced CT scan.
