Hello doctor,

This is regarding my father who is 88 years old and almost fully confined to a wheelchair. Though his vital body functions are all right, he has been lately getting panic attacks due to the fear of falling, even while walking a few steps. This is even though he has a male nurse and also my mother to help him at all times. He also seems to have lost interest in all things and just wants to lie down all the time. He has no appetite and has to be forced even to take a small quantity of food. Is there anything that can be done medically to help this situation?

Thank you for taking help from a psychiatrist. I can understand your situation and concern about your father. He may have major depressive disorder due to his chronic medical illness.

  1. He should do meditation daily at home, it will calm him and reduces anxiety.
  2. He should try to walk with the help of a nurse (as you mentioned) or a family member for 15 minutes.
  3. Ask him to spend time with friends or call them. Even ask him to sit with your children and spend time with them.
  4. Ask him to read or listen to good music.
  5. Spend time in nature, do not just stay in the room.
  6. Take enough good early morning sunlight.
  7. If possible, he must take lunch and dinner with his family, not on his bed or in the room.
  8. I suggest you give him a tablet of Escitalopram 10 mg daily in the morning after breakfast. It is an antidepressant. Consult your specialist doctor, discuss with them, and take medications with their consent.
