Hi doctor,
While masturbating in the shower with soap, I had a slight itch at the bottom left side of the penile meatus. I continued to masturbate in the next two days, and the tip became very irritated. I have been treating an irritated penile gland (feels like sandpaper at the tip when rubbing against clothes) with an antifungal cream for about 14 days. It is 98 % gone.
There is no discharge or pain, or itching when urinating or ejaculation. I live in a place with high heat and humidity. I also have a lot of sweating during sports-related activities. Is this a fungal infection, or could it be something else? Please help.

The changes are due to irritation with soap as well as due to frequent rubbing. Any soothing and moisturizing cream or lotion would help to resolve. Were there any white patches to suggest fungal infection? Did you have any sexual contact recently to suspect any infections?
We cannot rule out the possibility of superimposed fungal infection, as the fungus is usually present in that area. Trauma to the mucous membranes helps their proliferation, leading to worsening of the symptoms. The possibility of any other STD (sexually transmitted diseases) appears to be less if you have not had any recent sexual encounters.

I had protected sex before a few weeks. I used condoms for both oral and vaginal sex. I was careful and changed my condoms for every action.
There are no white patches, no lesions, no sores, no bleeding, no pain during urinating, no pain during ejaculation, and no itching. I was planning to get tested. I do not think that this is an STD.

Yes, it is unlikely to be an STD. It is likely due to irritation only. Usually, STDs affect the glans, and they produce sores, ulcers, white patches, and painful vesicular lesions. But nothing was there, and also it had not followed after the sex. So I think there is no need to worry about it. However, there is no harm in getting tested. Just masturbation will not cause any STD, other than irritation, trauma, and fungal infection.
