Hi doctor,

I am a 17-year-old female. I have this problem for about two and a half years. I feel weak and fatigued to the point where I am not able to walk sometimes. It tends to show fluctuation for two days. It becomes fine on the next day and the weakness comes back again. I do not have enough strength to do my work. I feel tired. My legs and arms seem to be weak. I have done many tests and all the results came back normal. I have done MRI of the brain and spine, EEG, ECG, complete blood count, vitamin B12, thyroid function test, and lipid profile test. All the results came back normal. I consulted a neurologist and she advised me to do an RNS test for myasthenia gravis. Another neurologist advised me to do an anti-AChR and anti-MuSK antibody test. I have done the tests and the results came back negative. Do I need to do a repetitive nerve stimulation test further? Please suggest.

I do not think that you have myasthenia gravis because myasthenia gravis causes weakness of the biceps. You will not be able to open the eyes due to biceps of eyelids. RNS (repetitive nerve stimulation test) helps to diagnose myasthenia gravis. But your symptoms do not correlates with myasthenia gravis. I have gone through the report (attachment removed to protect patient identity) and HbA1c (hemoglobin a1c) is higher and it diagnoses diabetes mellitus. Please mention whether you have any increased thirst or hunger or increased urination at night. Please mention whether you have been diagnosed with diabetes earlier. Please mention whether you have a family history of diabetes. You can avoid doing an RNS test.

I manage diabetes by taking Glimepiride and Glucophage. Before three years, I was asthmatic and the doctor suggested me to take Prednisolone six tablets for five days. After taking it, I noticed that I was not able to walk well and started to have high blood sugar levels. I do not had my energy to do my work and I always sleep on my bed. At one point, I was using a wheelchair for two weeks. After that, I got better. Now, I feel tired all the time. When I do some work while having a bit of energy, I become weak again. I feel difficult to get back my energy after taking a rest. Please advise.

In my opinion, you do not have a diagnosis of myasthenia gravis. Please mention whether you are taking steroids. Steroid addiction can cause these symptoms. I advise you have strict control of your blood sugar. Please mention whether you have any numbness in your legs.

I do not have numbness in my legs. I am not taking steroids now. I have stopped taking it as the whole problem got started after taking it.

I have done kidney function tests, thyroid function tests, and vitamin B12. All the results came back normal. My slippers does not slip. Most of the time, I have weakness in my legs while walking. I can walk but I fall down suddenly. Sometimes, I will be fine for several months and I have weakness again. I was fine for ten months and then again I became sick. My mood seems to be fine. I do not have any sudden anger outbursts.

Please mention whether you have difficulty while getting up from the squatting position. Please mention whether you have any numbness in your legs. I have difficulty while lifting my arms above the shoulder. I do not feel difficult while getting up from the chair. I do not have any difficulty while getting up from the squatting position. I do not have numbness in my legs. I can walk well sometimes my knees goes down while walking. I am staggering.

You need to control your blood sugar levels as you have a high level of Hba1c. Please mention whether you have done an NCV (nerve conduction velocity) test. It helps to diagnose whether you have nerve damage.
