Hello doctor,

I have neck and shoulder pain, falling sensations, rapid heartbeat, low blood pressure, severe sicca symptoms, nose fluid, lack of air sensation, difficulty breathing, dry eyes, lack of saliva, and lack of sweating, body thermostat has broken. I feel either too cold or too hot, always have goosebumps on the skin, dry vagina, sexual dysfunction, hair and skin changes, and dryness.

Neurological symptoms such as pain, burning, numbness, lack of sensations, random pruny fingers, severe tinnitus, weight loss, tummy and digestion problems, weird circulation, and change in veins' appearance, nail problems, body shaking.

I suspected Sjogren's, but all my blood tests for thyroid, menopause are negative. Endoscopy and ultrasound show intact. So I believe it to be functional problems, and more things are getting worse every day. It feels that everything in my body has stopped or struggling to function properly. Having researched for months, it all points to autonomic dysfunction. I spent on different doctors and specialists and fobbed off as it seems I am in the difficult box. Before this, I was a normal 48-year-old woman and mother, and I am now a housebound wreck. Very few doctors know dysautonomia, although one suggested it as possible, then fobbed me off too. I would appreciate any answers.

I would like to know some details about your problems.

Since when your symptoms started? Also, please describe how your symptoms began and how new symptoms sequentially appeared and worsened with time? Do you have any history of falls while walking or a history of urinary incontinence or urinary urgency? How is your day to day mood? How is your sleep? Do you have an episodic feeling of impending doom and anxiety spells too? How is your appetite? Have you noticed any weight loss in recent times?

Please send reports of your all test, whichever done till now, as an attachment.

Investigations to be done

Postural measurement of BP. Tilt table test. NCV (nerve conduction velocity) of all four limbs ANA (IFA method). ENA (extended nuclear antigen profile). T3, T4, TSH (thyroid-stimulating hormone).

Differential diagnosis

Anxiety. Hyperthyroidism. Autonomic dysautonomia.

Regarding follow up

Revert with the above-mentioned information for better understanding and further judgment.

Thank you doctor,

All my neck and shoulder pain started before five years when I noticed low blood pressure and had falling sensations. Doctor thought it was vertigo, but in hindsight, I did not think it was. The same year I had a mountain biking accident on crutches for six weeks, no broken bones but soft tissue damage to the right lower back hip, and groin. I recovered from this and fell again within a few months. After a few days, I noticed a cold, but suddenly, I had no nose fluid anymore, and sweat had virtually disappeared and now is very sporadic. But I do not sweat. I also noticed that I lost my body thermostat. I feel cold when no one else was. I again had sensations of falling with low blood pressure. I fell ill at work and nearly fell off of my chair. After pestering nose doctors about where my nose fluid was gone, they found a small nasal papilloma removed (noncancerous). Within 24 hours, I could not cry tears anymore and got dry eyes. I then had a lack of saliva, hair, and skin becomes dry, and suffering from insomnia. I had Diazepam only low dose but went through horrendous withdrawal through bad advice. I also got a burning sensation on the top of my left leg, numbness in feet and hands, sensitivity to pain, and lack of sensory touch. I lost my appetite and sense of smell and taste, and even though I continued to eat lost two stone in weight. I could only eat a small amount and felt full. I also became photosensitive and intolerant to the sun and heat.

My CT scan is normal for the abdomen, pelvis, chest, etc. My upper endoscopy found nothing. Further chest x-ray is negative even though I am constantly breathless and feels like I cannot breathe properly anymore. Ultrasound of the salivary glands looked normal. I have a mucus retention cyst on the back of the throat. I had ANA, ENA, and immunoglobins done, which were negative. I had Vit B12 and D did which were within levels. I never suffered from anxiety, weight loss, or other health problems. My mood was very good until this all started, and now I am anxious. I have control of my bladder but have noticed a bit of leakage.

Your features does not exactly fit in features of dysautonomia also. With all these tests showing nothing, it is difficult to make a conclusion. May be you are having fibromyalgia with associated anxiety and depressive component. Still I suggest for a few tests which can rule out other possible causes too.

Investigations to be done

NCV all four limbs. Sympathetic skin response. MRI Brain and whole spine with contrast. RR variability test. Tilt table test to look for any postural drop in BP.

Treatment plan

Meanwhile, you can try tablet Etizolam 0.5 mg, Propranolol 20 mg, and Fluoxetine 20 mg BD.

Preventive measures

Start doing daily exercises. Jogging for 20-30 minutes/ day in the morning hours.

Regarding follow up

Revert with above-mentioned test reports and after two weeks of taking these medications.

Thank you doctor,

I cannot jog as I feel too breathless all the time and my heart beat is always really high for some reasons. I forgot to mention that above. As I cannot breath through the nose anymore and I have to mouth breath with an already dry mouth I am finding any exertion hard. Also I cannot understand why I have no body thermostat and cannot bear the sun. I can see how fibromyalgia would cause severe Sicca syndrome throughout my whole body. I am pretty sure my autonomic system is out of balance as my heart rate and bp is too and my whole respiratory system feels dry.

I have read over time symptoms of fibromyalgia and do not have many of them. Also I was not anxious or depressed until I got severe with this illness.

Your tests for sicca syndrome are also negative (negative ENA panel). However, it is not 100% sensitive test. Feeling breathlessness with difficulty in nose breathing but able to do mouth breathing may suggest some nasal cavity or nasopharyngeal obstruction (structural or dynamic).

You are right that all symptoms are not fitting in fibromyalgia also. Also, not fitting completely in dysautonomia.

For that, I can suggest you undergo a few extra tests (other than previously mentioned):

HRCT chest and neck (including nasopharynx) to rule out lung involvement or nasopharyngeal obstruction.

2D echocardiography to rule out cardiac causes of breathlessness.

Please go for an ENT examination to look for any lesion inside the nose and a Rheumatologist consultation.

Investigations to be done

NCV all four limbs. Sympathetic skin response. MRI brain and MRI whole spine with contrast. RR variability test. Tilt table test to look for any postural drop in BP.

Preventive measures

If you find difficulty in jogging, you can do brisk walking or yoga therapy.

Regarding follow up

ENT examination to look for any lesion inside nose/nasopharynx. Rheumatologist consultation if they also not make any conclusion, you can give a try to medicines as suggested in my previous reply and revert to me after two weeks of regular medicine intake.

Same symptoms don't mean you have the same problem. Consult a doctor now!
