Hi doctor,

I am a 22-year-old male. I am suffering from pain in the anal area. I have a lump with a white top. It keeps on increasing and decreasing. I have a sticky liquid coming out of it and my anus remains wet always. I have this issue for three months. What to do?

You can have a possibility of perianal sinus or abscess. There is a pus collection around the anus. As you have this issue for three months, the other possibility is infection with secondary abscess formation. You can have another possibility of inflammation of the bowel in the form of inflammatory bowel disease. If it is a simple infection, then the treatment is straightforward and a longer and specialized treatment can help to control the issue in later stages. For now, I suggest you do stool tests and blood tests. Kindly attach a picture of it to guide you further. Please mention whether you have any changes in bowel habits, weight loss, bleeding in stools, abdominal pain, history of smoking, alcohol, any family history of similar illness, any personal history of exposure to TB (tuberculosis) patients, and any travel history. This information will help us to diagnose your problem and then treatment can be given accordingly.

The Probable causes

You seem to have an ischiorectal abscess, perianal abscess, or Crohn's disease with perianal disease.

Investigations to be done

You need to do a complete blood count, serum CRP (C-reactive protein), stool for fecal calprotectin, and stool for ova and parasites.

Treatment plan

Apply Xylocaine 2% gel on the perianal and anal region.

Regarding follow up

Have a follow-up with your investigations and information.
