Hello doctor,

I am a 40-year-old female. I have been having pain in the shoulder many times, which extends from the neck to the whole left hand. It pains me a lot. I have been suffering from this pain for more than six months. It pains me so much that I have no control over my body and get tears. Please help me get this pain out of me.

I understand your concern. The probable causes are cervical spondylosis or peri-arthritis of the shoulder. First, I need to ask a few things:

  1. Do you have any co-morbidity like diabetes or hypothyroid?
  2. Do you have pain in any other joints?
  3. Do you have pain just when you get up in the morning?
  4. Does the pain vary with neck movement?

You need to get a few tests to establish your diagnosis.

  1. X-ray of the cervical spine in anteroposterior and lateral view.
  2. X-ray of both shoulders in the anteroposterior view.
  3. Serum calcium.
  4. ALP (alkaline phosphatase).
  5. Erythrocyte Sedimentation Rate (Westergren).
  6. Random blood sugar.
  7. Creatinine.

I suggest the following medicines:

  1. Tablet Ultracet (Tramadol) twice a day.
  2. Tablet Calten D (Calcium carbonate and Vitamin D3) twice a day.
  3. Calcitriol (Cholecalciferol) sachet once a week for ten weeks.
  4. Tablet Preega M (Pregabalin and Methylcobalamin) 75 once a day in the evening. Please consult your specialist doctor, talk with them and start taking the medicines with their consent.

You may also require a cervical collar after investigations. You can follow these preventive measures:

  1. Keep the neck straight at all times.
  2. Do not use a thick pillow.
  3. Do isometric neck exercises.
