I popped a pimple accidentally, and now it is swollen and red and so big, and there is a lot of pain. Also, my marriage is coming soon; how much time will it take to cure?

I understand your concern.

I have gone through your history and the image provided from your end from the attached reports section (attachments removed to protect the patient's identity). It is a post-inflammatory reaction that has been caused due to trauma in the form of pricking up acne. They may be associated with redness, increased size, pain, tenderness on touch, burning, and itching. If not treated, they may result in the formation of post-inflammatory hyperpigmentation later on.

It is always advised to avoid habitual pricking of acne as it may result in the formation of inflammation and post-inflammatory hyperpigmentation which is relatively harder to treat as compared to acne itself. However, as you have mentioned, a function is happening soon; the most appropriate and faster treatment to help reduce inflammation would be to take an injection, the details of which will be provided below from my end.

My suggestion would be :

1. Skin rejuvenating face wash gel may be used externally to cleanse the affected area and face. The face wash may be used in the morning and at night. Apply a sufficient amount, massage in gentle circular motions for one minute, and wash off. It may be continued for months for better results.

2. Clindamycin and Nicotinamide gel will only be applied externally on the affected area. This medication is advised to be used in the morning after using the face wash. They help reduce inflammation and swelling, which are caused due to acne and due to post-acne reactions. They are suggested to be used continuously for months together for better results. In case of active acne as well, this medication may be continued.

3. Cleargel AP gel (Adapalene and Clindamycin) may be used at night after using the face wash externally on the affected area only. This medication being strong, is always advised to be used with caution. The smallest quantity possible of the medication may be taken, and a skinny layer of the medication may be applied to the affected area. They help in reducing the inflammation and the swelling which is associated with acne and which has been caused due to the inflammation post-trauma. However, in first-term users, they may sometimes result in irritation, redness, burning sensation, swelling, and inflammation. Therefore it is advised to be used with caution in the smallest quantity possible.

4. For faster recovery, procedures are the primary stay treatment and the only way to reduce inflammation and swelling, which is associated with post-traumatic inflammation due to pricking of acne. ILS, also called intralesional corticosteroids, use Triamcinolone injections which are placed in a minimal quantity and dosage on the affected area. Depending upon the size of the inflammation may need up to three or four injections 1 cm apart on the affected area. Usually, this procedure can be done in a single session and may not require follow-up sessions. They help reduce the inflammation caused by acne due to chronic pricking of acne. This is not an expensive treatment and is usually affordable. They are done at a dermatologist's clinic, so I request you consult a dermatologist to undergo the procedure. In certain conditions, however, and in the absence of the desired result, further sessions, either one or two, may be required as followed sessions to reduce the inflammation altogether. This is the most effective method for faster infection recovery, associated with the dramatic pricking of acne.

5. Tablet Diptase (Aceclofenac and Serratiopeptidase) may be used twice daily to be taken orally after breakfast and after dinner with water. They may be used for three days in total. They help in reducing inflammation and swelling, which is associated with post-traumatic pricking of acne, and would help in reducing the pain, which is associated with the same.

6. Tablet Pantop (Pantoprazole) 40 mg taken 30 minutes before breakfast and 30 minutes before dinner to help reduce the formation of acidity following the intake of the above medication. The medication mentioned in the fifth point, being a solid and anti-inflammatory medication, it is always to be consumed after having enough food, before which Pantoprazole tablet is to be consumed to prevent abdominal discomfort and acidity.

7. Capsule Doxycycline 100 mg twice daily to be taken orally with water after breakfast and after dinner along with Tablet Diptase. However, Diptase maybe stop after three days, but Doxycycline needs to be continued for seven days. They help prevent secondary bacterial infection and help in reducing the current active acne and the post-traumatic swelling and inflammation associated with the same. It needs to be noted that since this is an antibiotic, it needs to be made sure that there is no previous history of allergy to any antibiotics before consuming the capsule. This medication may be avoided in case of any previous history of allergy. Also, since Diptase is taken along with this medication, and both are strong medications, having Pantop and consuming food reasonably is essential to prevent any abdominal discomfort associated with solid medications. Therefore, the Pantop tablet may be continued for seven days till the course of Doxycycline is complete.

8. Avoid oily and fried food, milk and dairy products, red meat, and sweets, as they trigger the formation of acne and increase the possibility of inflammation in the existing swollen area. Instead, consume more fruits, vegetables, and fiber and consume at least 12 glasses of water daily in a 250 ml glass size.
