Hi doctor,

I am 30 years old, and my wife is 29 years old. We are trying for a baby for the last eight to nine months, but no positive result yet. I do not drink. I smoke very rarely, one or two a day. She is having some PCOD problems. During ovulation days, I sometimes become very nervous and thus fail to have intercourse. I am attaching the test results for your reference. Please suggest medicine to overcome nervousness.

Only your semen analysis was present in the attached file (attachments removed to protect the patient's identity). The TVS (transvaginal ultrasound) is not seen. Let me explain to you about the semen analysis. Overall your semen analysis is normal (good count, good motility, and normal morphology), but a slight increase in pus cells (one to two pus cells is normal, yours is around three to four). This may be due to a mild infection. So that can be treated with a course of mild antibiotics. I cannot say about your wife's scan as I have not seen it. So kindly send it to me for me to review. If PCOS (polycystic ovarian syndrome) is the only problem, conception will happen when she ovulates. These are some of the things you can do to increase fertility:

1) Quit smoking completely (this is for you, even one to two per day is harmful to the sperm).

2) Track your wife's period and find the fertile days by calendar method and by basal body temperature checking and ovulation kits (If you have any doubt regarding this, please ask, I will explain it in follow-up).

3) Be comfortable and stress-free. To perform better, you need to have a calm mind. Do not worry about conceiving. During the intercourse, try to be in the moment, as you have no problems. Otherwise, it will not be useful to prescribe drugs.

4) Both can take supplements containing antioxidants, multivitamins, and amino acids. This will aid in conceiving.

5) Good diet and regular exercise will help with stamina.

6) Do not masturbate during the fertile window.

7) Intercourse during fertile window should be either had once daily or once alternate days for sperm to replenish.

I have gone through the TVS. It shows no PCOS changes. It is a normal USG (ultrasonography). So a little bit of planning and supplements can help you both conceive easily. Kindly follow the instruction given before. Also, can I know how long have you both have been married? The stress to conceive makes conceiving difficult so kindly remain stress-free. I hope this was helpful.
