Hello doctor,

I have been taking the antibiotic Ciprofloxacin 500 mg twice daily for prostatitis for the past 19 days. I was feeling very ill before taking the medication and was urinating ten to 15 times in an hour.The prostatis has improved, but I feel very unwell, tired, and achy after taking the antibiotics. Is it normal to feel like this after taking antibiotics? Kindly suggest.

Probably in your region, it is still working, but we usually avoid Ciprofloxacin due to resistance. Anyways, prostatitis requires three to four weeks of treatment. The reason for your symptoms could be the antibiotics, as they are known side effects of the fluoroquinolone group of antibiotics to which Ciprofloxacin also belongs. Muscle and tendon aches are common. Repeated urination also must be contributing to overall exhaustion. It is always advised to drink plenty of water while on antibiotics. Some vitamin supplements can also help. Is it the first episode of prostatitis? What is the underlying cause of prostatitis in your case? As the frequency of urination in your case is quite high, there are options to use other medicines to improve it. Please speak to your doctor about it. I hope this helps.

Hello doctor,

I had Prostatitis six years ago, so this seems to be a flare-up. I can explain a little more history. I was advised testosterone replacement gel as I have low testosterone. About two weeks into the course, I started feeling unwell with bladder pain, testicle pain, tiredness, lower back pain, etc. I was first diagnosed with a UTI and given antibiotics. However, for that week, it became worse. I had a complete examination by my doctor, who then changed the diagnosis to prostatitis. I was told to stop the testosterone replacement gel. I have been on antibiotics for three weeks, and the pain and urinating a lot have gone now. However, I am still feeling very run down and tired. Do you think these symptoms could be due to the tablet Ciprofloxacin antibiotic or possibly withdrawal from the testosterone replacement gel? I used the gel for almost two weeks. Kindly help.

I can understand your concerns. The symptoms that you expressed are significant. Not antibiotics alone can contribute to it. There is something else behind the symptoms. Testosterone replacement gel cannot cause this. Have you ever done your routine blood counts in recent times? If not, I suggest doing it once. It is difficult to comment without examining the patient. I suggest you see your doctor for a detailed evaluation.

Erythrocyte sedimentation rate 5 mm/h.

Urea and electrolytes values are -

  3. Serum creatinine level 0.87 mg/dL.
  4. GFR calculated abbreviated MDRD > 60 mL/min.
  5. EGFR COMMENT 1 eGFR should not be used in patients with acute renal impairment.

Your blood test report is alright, nothing to worry about. Prostatitis is very slow and responsive to treatment, which is why long-term antibiotics are usually prescribed. Your exhaustion could be due to an effect of antibiotics (most probable) as tablet Ciprofloxacin is a broad-spectrum antibiotic. I suggest you include some over-the-counter multivitamin supplements as well. If you are not tolerating the medicine (which can happen), your doctor will change the medication. I hope this helps.
