Hello doctor,

I got an unprotected blowjob done by a spa therapist. She told me that she does not have any STDs. However, after 17 to 18 days, I started getting small red pimples on my penis head. Is it a symptom of HIV? I am anxious.

Oral sex can transmit infection. Kindly share a picture of the affected area. If it is an STI (sexually transmitted infection), I suggest you to get the below tests done:

  • TPHA (treponema palladium hemagglutination assay).
  • HPV (human papilloma virus) DNA PCR test.
  • HSV (herpes simplex virus) 1 and 2 IgG.
  • HSV 1 and 2 IgM, serum.
  • Chlamydia Trachomatis IgG.
  • HIV (human immunodeficiency virus) 1 and 2 antibodies screening test.
  • HbsAg.
  • If there is itching, I suggest you use ointment Candid B (Clotrimazole and Beclometasone) for local application and tablet Cetzin (Cetirizine) once at night. If it is a bacterial infection, you can use ointment T bact (Mupirocin) and tablet Doxy 100 mg (Doxycycline) twice daily for five days. Consult a specialist doctor, talk to him or her, and start the medicines with their consent. Also, you must wash your genitalia twice daily with normal water. Do not use a heavy pH soap.

There is no itching. Also, the red spots appeared only today. Is there any chance of getting HIV through oral route low? Also, can it be any other STI?

The chances of HIV (human immunodeficiency virus) are low. Hepatitis B, HSSV (herpes simplex virus), fungal infections, and HPV (human papillomavirus) are the other infections that can occur.

Any initial guess, according to you, what could be the reason behind these red spots? As you have seen, the affected area. Could it be herpes? I will get the testing done. As the exposure was around 18 to 19 days back, should I get an antigen-antibody test for HIV, or getting an STD panel will help?

It looks like a fungal infection. Herpes lesions are small and round with or without fluid collection. I suggest you to go for STD (sexually transmitted diseases) panel. Do share the reports for further guidance.

I did the HIV 1 and 2 antibodies and the HbsAg tests. Both are non-reactive. The red spots have become flat, and some have diminished. Also, I have noticed two tiny white fungi-like structures on the foreskin that have turned red after cleaning with water. Is it herpes? Also, the skin on my penis is uncircumscribed. Can you please suggest an ointment for this?
