Hello doctor,

I have recently been diagnosed with anxiety for my symptoms. Randomly, I will get severe stomach pains, and my hands and feet will get warm. I get lightheaded and then go to the restroom. My blood pressure can spike to 160/100 mm of Hg. Pulse of 130 to 140 beats per minute. After about an hour, the elevated levels return to normal, but I feel awkward for the rest of the day. I feel light in the head or off but not dizzy or anything. I also get clear urine for a few bouts after this happens, and I often yawn. All tests I have done came back good. X-rays are good, heart tests good, no blood clots, normal thyroid, normal glands. I also did a 24-hour urine test, and everything came back well. Nothing out of the ordinary except fatty liver. I do not think it is anxiety since my mother started to experience the same thing recently and ended up in the ER multiple times, much like myself when it first happened. Currently, I am taking the tablet Buspirone 5mg and tablet Lexapro 20 mg. Any thoughts on if this is a stomach issue or what?

I read your query and can understand your concern.

I have reviewed your case history and attached laboratory investigations (attachments removed to protect the patient's identity). I will discuss your symptoms individually and return to stomach pain at the end.

1) Elevated blood pressure and heart rate are features of distress. When you are distressed by stomach pain or anything else, these two parameters can go high, so nothing specific is related to that.

2) Next, about clear urine. If you are passing urine again and again or drinking water, your urine will be as transparent as drinking water. Again, this is episodic. The organic disease will not cause episodic symptoms. Usually, there are persistent symptoms, irrespective of day and night, and irresponsible of your mood and surroundings.

3) Your laboratory reports are good, but you cannot get logical reasoning for your symptoms. So then, I reviewed both medicine that you are currently taking. Both are related to anxiety, and usually, they will take some time to have optimal effects. It would be best if you were on some stomach medicines to calm down your symptoms, like a capsule of Pantoprazole 40 mg (proton pump inhibitor) once daily a half hour before breakfast, syrup Gaviscon (antacid) 2 tablespoons full after every meal or as per need for abdominal bloating, and pain. Similarly, if you feel cramps or colic pains, I suggest you take the tablet Mebeverine (antispasmodic) 135 mg as per need. It is an excellent anti-spasmodic medicine. However, please consult a specialist, discuss with them and start taking the medications with their consent.

4) You have given similar history related to your mother as well. I want to ask, is there any recent stress or difficult situation at your end that can cause worsening symptoms for you and your mother? Do you or your mother had a history of anxiety or depression in the past, or were you on any anti-depressants before?

I would suggest that your laboratory reports are standard, so give yourself some time with the above medicine, and I am hopeful you will be okay soon. Meanwhile, avoid stress or uncomfortable conditions, try to have home-cooked healthy meals, and spend some time with your friends and family.
