Hi doctor,

My friend had a heartbeat of 204 bpm yesterday, and today she had 104 bpm. She is under treatment in another country. Doctors are unable to find the reason. CT scan, BP, etc., are all fine. I need your advice on what could be other possible reasons. Thanks.

What were her symptoms during the heart rate of 204 bpm? Could they perform an ECG during heart of tachycardia? Was this the first time or she had episodes prior to this? Now the common possibilities here are arrhythmias like PSVT or Atrial fibrillation. These are common arrhythmias causing such picture, and are treatable. So that's a good thing. In order to confirm the cause for sure, it's important to have an ECG during the tachycardia. So share the ECG if you have or else try to obtain an ECG during tachycardia. For that, patient should immediately rush to nearby hospital during symptoms for an ECG, as it may not persist for long. Alternatively she may buy home ECG device like kardia device and obtain frequent digital ECG especially when she notices symptoms, with the hope of capturing the abnormalities in the ECG. If she is getting frequent episodes, say few times a week then 24 hour holter monitoring is an option. So important thing is that to have an ECG available during tachycardia to confirm the cause, or else we might have to go for invasive tests like electrophysiological studies. Also with all such tachycardia, it's important to do routine tests like echo, thyroid profile etc. Hope this helps you and get back if you have any doubts or more information to share.
