Hi doctor,

I am a 28-year-old unmarried male. My spermatic cords and other nerves connecting both the testicles pain considerably for prolonged hours. Especially after getting aroused, pain is there (usually it happens when I get aroused but did not discharge my semen). Both the testicles feel tender. Sometimes lower abdomen also develops some pain during this time. Also, sometimes I feel a sense of nausea during this time. I never had intercourse, and I masturbate once in a fortnight or so for the last three to four months. Earlier, I used to masturbate daily. Kindly help me.

I can understand your concern.

A lot of reason for your issue, it can be:

1) Prone masturbation can cause pain due to injury.

2) Holding or prolonging orgasm during masturbation.

3) Epididymitis is Inflammation of the tube, back of the testicle that carries sperm due to infection (both sexually and non sexually transmitted), diagnosed by urine complete test, ultrasound pelvis.

4) Hydrocele can cause swelling.

5) Hernia can also cause swelling, but it will be on and off.

6) Kidney stones can cause abdominal pain too.

7) Infections can cause fever, discharge from the penis, etc.

8) Testicular torsion.

9) Urinary tract infection (UTI).

10) Any previous surgical procedure.

11) Benign or malignant lesion.

Kindly share more details regarding the above reasons. For now you can use tablet Zerodol P (Aceclofenac and Paracetamol) for pain. You can do an ultrasound scrotum and pelvis for better understanding and work on management. I hope this was helpful.
