Hi doctor,

I am a 34-year-old man with no significant past medical history. Now I have painful white lesions in my throat for ten days. I am unable to eat. I started Amoxicillin for seven days and had no change. I went to the ER two days ago and started on Clindamycin. It is excruciating. I cannot eat. Bacterial cultures were taken two days ago, and they were negative. Mono quick test was also negative. I cannot get to see an ENT until next week. Do you think it is fungal or viral? I will attach photos for your reference. I have no risks of HIV, and I am not immunocompromised.

Though the pictures you have sent are not very clear (attachments removed to protect the patient's identity), it seems you have uvulitis with follicular tonsillitis. This kind of infection should resolve with Clindamycin, so you may continue taking it until you meet an ENT (ear, nose, and throat) specialist. When you meet your doctor, request him to take a throat swab and send it for culture sensitivity and gram stain. Please do not stop any antibiotics abruptly. If you feel better after taking Clindamycin, you can even avoid an ENT appointment. Also, start taking anti-inflammatory medicines like Paracetamol 1 gm thrice daily for three days and decongestant like Phenylephrine for five days. Do not forget to take proton pump inhibitors like Rabeprazole once daily on an empty stomach (because all these medicines will cause gastritis). Avoid spicy and cold food items.

1) The bacterial culture is negative, but sensitivity reports are essential to know which antibiotics you are sensitive. 2) Sorry to say, but the pictures you have sent are not clear, I can see the infected uvula, but tonsils are not clear (it looks like follicular tonsillitis, though). Please add a torchlight and try to send a picture of the tonsils. 3) It does not look like a fungal infection in the picture. It can be viral with a superseded bacterial infection. 4) Since you have a slough over the uvula, and it looks like the follicular tonsillitis antibiotic course is a must. However, you have already taken Amoxicillin for a week and Clindamycin for two days, and there is no relief in your symptoms. So I suggest you higher antibiotics like LINID 1200 mg (Linezolid) in a divided dose per day for seven days along with anti-inflammatory, decongestant, and proton pump inhibitor. 5) It is wise to get a culture sensitivity test if you do not start on any antibiotics and want to wait until you meet ENT doctor personally.
