How to treat truncal and facial acne?

Hi doctor,

I may have a yeast build-up because I recently had folliculitis on my chest, stomach, legs, especially thighs, butt, back significantly lower, upper arms, and belt line. My face has been breaking out worse, and neither topical Aczone nor oral Acticlate seem to help as they once did, and putting Epiduo on my back, and upper arms are not working either. Also, the bumps on my chest, thighs, and butt resemble a rash. Is it fine to put Lotrimin ultra (Butenafine hydrochloride cream 1 %, antifungal) on my body and face? Have you seen this clear up the body and face before? Or are there other suggestions that you recommend?

With your descriptions and given pictures (attachments removed to protect the patient's identity), you have facial and trunkal acne. The causes of trunkal acne are tight-fitting exercise clothing that traps sweat next to the skin on the back and blocks pores may contribute to outbreaks. People should consider wearing loose-fitting clothes, especially during a heavy gym session.

Showering after sweating and using gentle cleansers applied with the fingertips can help to reduce the risk of acne breakouts on the back.

Other tips for reducing the risk of back acne breakouts are:

  1. Washing after sweating and using fingertips to apply a gentle, non-abrasive cleanser.
  2. Avoiding products that irritate your skin and not scrub the affected areas.
  3. Avoid popping, picking, or squeezing spots because this may lead to them spreading and scarring.
  4. Staying out of the sun and avoiding tanning beds because damaged skin is more prone to acne.
  5. I suggest you start using 1 % Clindamycin gel during day time over pimples and 0.1 % Adapalene gel (mention the composition in the bracket) at night hour over the whole face (dark spots) for around two to three months. You may develop a little bit of a burning sensation with Adapalene gel. Hence, you should use it in small quantities for the trunk. You can use 5 % Benzoyl peroxide twice daily for the same duration. In addition, I suggest you take the capsule Doxycyclin 100 mg once daily for one month.
  6. For cleaning your face, you can use soft foam gel face wash, and for trunk use, moist body wash.
  7. You have to do certain dietary restrictions like not taking spicy, oily, or junk foods, milk products, and non-vegetarian foods. Drink plenty of water to keep your skin hydrated.
  8. All medicine should be taken after consulting your nearby dermatologist
