Hello doctor,

I underwent varicocele surgery, which was done on the sack of the left testicle one month back. It was a 1.96 inch cut, and the ball was placed low. I can contract it minimally, but I do not know if it is because of the wound, the hard vein or spermatic cord in the bottom of the left testicle, or if the doctor had cut the muscle. The doctor said it can take up to six months for the veins to disappear. Can the muscle, or the skin regenerate or should I do something about it? How to accelerate the recovery? It still hurts when I walk. I am taking tablet Ibuprofen 600 mg once a day. Will it help? I am worried about it, and I do not see this method done elsewhere, only the one with a cut at the base of the penis.

This is subinguinal microscopic varicocele ligation. It is the best procedure to be done for varicocele. Actually, it should be done by cutting the skin and the spermatic facia, then exposing the spermatic cord using microscope, and veins should be ligated. Healing takes up to three months. During healing, what we look for is a wound. I can see no infection, and also the size of the testicle on that side is normal. It means you had a good surgery, and the healing is good. You can expect hardness, mild inflammation, and pain during healing but it will be fine. Semen analysis and ultrasound scrotum should be done after three months to look for improvement.

I think subinguinal surgery is not the one done above the testicle. The doctor did not use a microscope. Did the doctor cut the cremaster muscle or muscles off the testicle while cutting the skin, and which is why it is lower? Can this regenerate with time? The wound had been sutured for two weeks, but the bleeding was present. Earlier, it was with a small thread out, but now it seems it is under the skin, and the wound has healed.

A subinguinal incision is done above the testis to lower the superficial inguinal ring. Yes, the cremasteric muscle will heal. Regarding the use of the microscope, sometimes I use it and sometimes I do not. Instead, I use a doppler probe. Stitches used are vicryl which will dissolve with time.
