I am overweight. Please explain my blood sample reports.

Hello doctor,

I am a 31-year-old male weighing 220 pounds and standing 5 feet 8.5 inches. I have no health issues except being overweight. I had quit smoking and it’s been almost three years now. I consume alcohol on regular basis. Recently, I had taken a general complete blood and urine checkup. Certain readings on my blood sample are not normal. They are as follows:

  1. Lipoprotein: 43 mg/dL.
  2. Cholesterol (HDL): 55.2 mg/dL.
  3. Cholesterol (LDL): 125 mg/dL.
  4. Lipoprotein: 43 mg/dL.
  5. GGT: 68.8 U/L.
  6. Homocysteine: 25.3 µmol/L.
  7. Hs-CRP: 9.41 mg/L.
  8. Vitamin B12: 147 pg/mL.
  9. 25-hydroxyvitamin D: 20.5 ng/mL.

Please advise regarding my report.

I have gone through your reports (attachment removed to protect patient identity). Your reports are fine with some derangements.

Firstly, your LDL (low-density lipoprotein) cholesterol is borderline elevated, due to which lipoprotein (a) is elevated. So, since it is borderline elevated, there is no absolute need for medicines.

You should follow a healthy lifestyle, such as having less high-calorie food, a low-oil diet, and a spicy diet. Less salt, a light dinner, and avoiding rice. Regular exercises like running for at least 30 minutes a day.

Repeat the lipid profile after eight weeks and see the response. If there is a response, then we will continue the same, and if there is no response, then we will add medicines. If you think you are already having the best possible healthy lifestyle, then you should start on the tablet Rosuvastatin 5 mg at bedtime. Your HDL (high-density lipoprotein) is fine.

GGT (glucose tolerance test) is borderline elevated, and due to obesity and fat deposition in the liver and alcohol intake, weight loss and the above lifestyle will help. Also, alcohol consumption should be reduced. Vitamin D is low, so you should take a tablet. Take Uprise D3 (Cholecalciferol), 60,000 units, once a week for eight weeks and then once a month for six months after a weekly course is finished. HsCRP (highly sensitive C-reactive protein) is also elevated due to obesity and will gradually settle.

Next is elevated homocysteine, which is likely due to low vitamin B12. So you should have tablet ME12 (Methylcobalamin, Folic acid, and Pyridoxine) once daily for eight weeks for that. Also, get a folate test done to look for the cause of elevated homocysteine.

Thank you for your reply.

I appreciate your prompt response. As per your advice, please let me know if I have understood correctly. I should be taking the following medicines:

  1. Uprise D3 60,000 units once a week for eight weeks.
  2. ME12 tablet daily for eight weeks.

Also, please let me know if I will be diabetic in the future. I have no family history of diabetic patients. I just want to be sure that I am not prone to getting affected by diabetes or other cardiovascular diseases, according to my report.

Yes, that is correct for both medicines. Being overweight is a strong risk factor for diabetes, so you are certainly at risk of developing diabetes and hypertension in the future. So you should focus on diet and exercise in order to achieve weight loss.
