Hello doctor,

Is it possible to get herpes on the penis from masturbation? I have a small white mark on the head of my penis and I get cold sores on my lips after two days.

I am currently on Phenytoin, Clonazepam for epilepsy, Tamsulosin and Dutasteride for enlarged prostate.

Do you have papules, vesicles, or crust? Pain in lesion? Burning, tingling, or ulcer? Is it a primary episode or recurrent? Any lesion on the vermilion border of lips? Or genital lesions?

While I do have some of the problems. Above my recurrent cold sores, it is the white mark on my penis that concerns me a lot. It does not irritate, tingle or burn and, if it is herpes, I must have got it from saliva or touching my cold sore then masturbating two days before there were any signs of the cold sore. Is this possible and does it look like herpes on the attached photo?

No history of balanitis, no itching and I have not had a test. Is it possible to get herpes on the penis from masturbating two days before the cold sore shows?

Herpes is not caused by masturbation. So it is not the cause. You can get an injury or traumatic lesions from excess rubbing, scratching, or harsh handling.

When herpes (viral infection) occurs it begins with itching sensation, papule, burning, vesicles, pain, tingling sensation, and crust formation. Usually the lesion heals within 10 to 14 days.

If the lesion is due to injury use Fucidin cream twice daily for seven days. Take tablet Zifi (Cefixime) 200 mg twice daily for five days. If herpes (confirmed by lab investigation) HSV1 or HSV2 take tablet Acyclovir 400 mg three times in a day at four hour interval for five days.

Maintain proper genital hygiene. Avoid excess scratching and rubbing of lesion. It can cause secondary bacterial infection. Wash your genital after sexual intercourse. Use neutral non-soap cleanser to clean genitals. Use warm water. Retract the foreskin and clean glans daily.

I am not diabetic. I have not had any of the symptoms you describe for herpes and have not had intercourse since March due to the lockdown. Do I take it that it is more likely to be damaged from masturbation than herpes?

Yes, it is trauma or injury from rough handling (masturbation). Apply Fucidin cream twice daily for seven days. Avoid rough handling.

When I masturbated, I used my saliva as lubricant. Could I have got herpes through this, or do you still think it is due to rough handling? Bearing in mind, the masturbation occurred on Sunday morning, and the cold sore did not start to show until Tuesday.

Is there a history of herpes simplex 1? Any reports or past episodes?

You should get tested for HSV1 and HSV2 to clear any doubts.

Then it is possible to get an infection. Have you undergone lab investigation for herpes? What are the results?

If reactive, then I suggest you take tablet Acyclovir 400 mg thrice daily (at 10 am, 2 pm, 6 pm) for five days. Apply Fucidin cream twice daily for seven days (on lesions).
