Hello doctor,

I have fissure from last three months and the doctor recommends Anobliss cream and Lactulose solution and also sitz bath. Now, the pain is gone and it is a long time but the fissure remains there. What is the next step?

Your doctor has given you the correct prescription for your problem. If the problem is still persisting, I recommend that you continue the application of Anobliss cream and taking Lactulose solution. However, if there is no pain or bleed, you can stop the sitz bath for now.

Your doctor is a better judge to check the state of your fissure. It can only be determined by a clinical examination. For chronic cases, there is an option for surgical repair but that cannot be decided without a physical examination. I recommend that you get yourself examined every two to three months to know the progression of your problem.

In the meantime, continue taking the high fiber diet and Lactulose to avoid constipation. Adequate intake of water is also very important.
