Hello doctor,

I am a 57-year-old male and was suspected of having fungal infection after sensing slight irritation and swelling in the glans penis a few days ago. I applied Candid cream on the whole penis for one week and took a Terbinafine tablet after consulting online with a sexologist. The burning sensation in the penis is still there even after a week. So I visited a urologist who ruled out the fungal infection and prescribed Oflomsc 400, Afdura, and Urispas. He also suggested some tests like USG, CBC, and urine culture. As per the urologist, nothing significant, such as, a UTI was found, only mild urinary bladder thickening with insignificant PVRU. I have taken antibiotics for the last ten days, but the burning sensation in the penis is still there though it has slightly decreased. During this period, I had no abnormal discharge from the penis, no swelling, no associated pain or fever, no sores, and no chances of having an STD. I am passing normal urine. Itching in the scrotum usually happen due to contact with soap and related agent, some times I use antiseptic and over-the-counter topical agents. I am really worried. Kindly suggest.

How many times do you get up at night to pass urine? How is urine flow? Do you need to strain or rush to the washroom to pass urine? How are your erections? Does it pain during ejaculation? Do you have constipation, bowel habits, or PSA (prostate-specific antigen)? According to the reports attached (attachment removed to protect the patient's identity), 17 gm prostate with mild thickened bladder and no UTI (urinary tract infection) is seen. Hence stop taking Oflomac (Ofloxacin) and Urispas (Flavoxate hydrochloride). I suggest you take the tablet Alfuzosin (Alfuzosin hydrochloride) instead of a combination with Dutasteride (Dutasteride 0.5 mg plus Tamsulosin 0.4 mg). Also, take the tablet Solitral (Alfuzosin with solifenacin) at bedtime and no other drug for five days. Please consult a specialist and take the medications with their consent.

Hello doctor,

I do not need to get up at night for the washroom. I do not strain or need to rush to the washroom. My erections are fine, and I do not have pain during ejaculation. I have been suffering from constipation for the last two months, earlier most of the time, my stools were loose and were three times a day, but now they are hard. I have not done the PSA test.

Except for burning in the penis, you do not seem to have any significant urinary symptoms. For burning sensations, I would suggest tablet Alfoo (Alfuzosin) 10 mg HS (hour of sleep), tablet Mirago-S (Mirabegron and Solifenacin) 25 or 5 mg after breakfast, and tablet Pregabalin plus Mecobalmine HS (hour of sleep) for ten days. Perineal exercise and regular sexual activity are important. Perineal exercise should be done daily once for three months, including peri-anal squeezing and relaxing fifteen times. Consult your specialist doctor, discuss with them, and take medications with their consent. After ten days of medicine and exercise, kindly update me on the effect.
