Hello doctor,

Recently (within the past month) my fiance had flown back to a different city where she is now currently having to live, and I ended up masturbating for a rather long time about a week after she had left. The time that this lasted for, I would say was anywhere from three to three and a half hours. This is far longer than I have ever masturbated before by a substantial amount, and when I finally allowed myself to climax I noticed that I had some pain in my penis and it was still kind of swollen. Initially, this did not disturb me, but then the swollenness did not go down immediately and only after about an hour or so did it finally subside. I had taken some Ibuprofen immediately when I noticed it may have had inflammation. And I just figured that it would go away overnight, but it did not. I have now had pain in my penis on and off for the past two weeks since the incident and it is really starting to concern me. I know there are certain things that can happen. And Peyronie's came to my mind. As of now, if and when I get an erection, it seems like my penis is bent to the side slightly. I guess my biggest concern is if it was the beginning of Peyronie's disease, would it develop slowly as the scar tissue developed, or would the bend be more abrupt? I currently do not have health insurance as I have not needed to go to a doctor for years, and I do not know what to do at this point. I have pictures showing the bend and also what seems to be some sort of bump or vein on my shaft that was not there before.

Had you taken any erection enhancing drug so that your erection lasted long? It should not be left erect for hours, like four hours and more. The image shared (attachment removed to protect patient identity) is an acceptable shape of the penis. I do not think you should be concerned about Peyronie's disease. Veins or slight swelling seen are also normal.

Hello doctor,

I have not taken any sort of medication that would cause me to have a lasting erection. I simply engaged in "edging" when masturbating and did it a lot longer than I normally ever would. The pain is still here nearly three weeks later now and I am still very concerned. I understand that the curvature would be considered normal, except that my penis was never curved like that before. That is why I am concerned that it could develop further into Peyronie's if the scar tissue continues to form as it heals. I am at a loss right now, and I really do not know what to do. Is there anything that can be done?

How are your erections during masturbation now? If they are normal, then there is nothing to worry. This pain and curvature as described by you does not seem to be Peyronie's disease to me. I would suggest not to label as Peyronie's disease. Just that, while masturbation do not bend it unnaturally when erect. For now, the issue is: Bend - need observation as it is very mild and natural. Swelling - observation is not anything of concern. Pain - a dull aching pain if bearable will settle with time. I suggest you take tablet Ultracet (Tramadol) or Hifenac (Aceclofenac), if severe. Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent. Erection - expected to be normal.

Honestly, I have not masturbated since for fear of causing any further damage to myself. I do seem to get normal erections though, but they also seem to go away quickly or are not sustained. Is there any reason why a bend would suddenly appear after having caused some sort of trauma to myself? I am still concerned that it is not the same way it used to be. Is it possible that I damaged it from sleeping on my stomach on a harder surface and then having an erection? I recently also had to sleep on a carpeted floor for a few nights due to a change in living situation.

The penile tissue is so tough that sleeping on a hard surface and erection in sleep mostly does not cause any damage. It is a sudden jerk or hitting the penis like accidentally in the thigh instead of vagina. That amount of sudden jerky trauma may cause some damage. But, it is unnecessary to stop masturbating. Now that you are sure that it was not curved before and now we have the picture it would be reasonable to monitor this curve whether it progresses over months. Take once a month picture and compare. For me, who has seen you for the first time at present, I consider you normal. Having experienced natural sex with a girl you may face some erection difficulty in masturbation. But, I am sure while having sex you will have the same normal erection.
