Patient's Query

Hello doctor,

I went through the lighter course of COVID-19 last month. I am fully recovered now for over a month. But my D-dimer lever is still up. Since the start of COVID until yesterday, it was fluctuating. The cut-off is 0.53mg/l, but I have fluctuating levels of 0.78, 1.17, 0.68, 0.74, and 0.74, and the last one two days ago, it was 0.91. I have been taking Aspirin to protect 100 mg. I did an EKG and echocardiogram of my heart yesterday, and everything is normal. All blood work are normal besides cholesterol, a little elevated on the borderline.

My question is, should I take Heparin for seven days (as per one doctor's suggestion), or should I stay on Aspirin for another month (like four other doctors suggested)?

Hello,

Welcome to icliniq.com.

You should stay on Aspirin for a week or longer. Add an anticoagulant, preferably Rivaroxaban 20 mg once daily for at least a couple of weeks. It seems you are having a low-grade compensated DIC (disseminated intravascular coagulation). Due to COVID-19, you might have been lying in bed for a long time. This might have induced some intravascular coagulation, also known as DVT (deep vein thrombosis). Get a color Doppler ultrasound of your vessels of extremities and upload the report. Also, get prothrombin time and aPTT (activated partial thromboplastin time) tests and send us the reports.

Patient's Query

Thank you doctor,

Should I stay on Aspirin for a couple of weeks and take the rest later? Here is blood work from two days ago. Is it possible to get an elevated D-dimer from cold as I have a cold for two days?

Hello,

Welcome back to icliniq.com.

You have got borderline high hemoglobin as well. Better take Aspirin and anticoagulant prophylaxis such as Rivaroxaban 20 mg a day. Get color Doppler ultrasound of legs for any DVT.

Same symptoms don't mean you have the same problem. Consult a doctor now!
