Hello doctor,

I am a 42-year-old female in good health. Last night, I had a very sudden violent episode of diarrhea, which had quite a bit of blood in it. I am quite worried about this, whether it could be something serious. I have had no more episodes since last night but have a very gurgly feel.

Did you take a picture of that? And did you eat something uncertain in the last couple of days, especially pork or beef? And do you have a fever? Does it cause pain while passing stools? And do you have constipation previously?

This one episode of diarrhea with blood can be due to an acute infection. Usually, it would resolve on its own. But to be very sure, if we are not missing something else, I want you to provide me the above information, and I suggest some testing to rule out infections.

Just keep taking oral rehydration solution dissolved in water and a healthy diet. Avoid juices and milk for now.

Investigations to be done

CBC (complete blood count). Stool for detail report and ova parasites. Stool for calprotectin blood. IHA (indirect hemagglutinin) titer for E. histolytica.

Thank doctor,

I am not aware of having eaten anything uncertain but did have a very gassy stomach the day before the episode and had some acid reflux, which I put down to some acidic fruit juice. I do suffer from constipation regularly and think the last time was around a week ago.

I did not take a picture, unfortunately. It was not painful but was uncomfortable, and no pain afterward. My temperature is normal at 36.7 degrees.

So it seems there is nothing serious in that. It can be piles induced or an infection. But both will settle down with the fiber, ORS, and a good diet.

Additionally, take capsule Omeprazole 40 mg (proton pump inhibitor) half an hour before breakfast once daily for a week, tablet Metronidazole 400 mg thrice daily for five days, and tablet Daflon 500 mg (diosmin ands hesperidin) thrice daily for five days. Get the tests done and let me know in three to four days about your symptoms.

Same symptoms don't mean you have the same problem. Consult a doctor now!
