Hello doctor,

I am a 23-year-old female, and for the past 17 months, I have been overcome with extreme exhaustion. Small tasks wear me out. This has been accompanied by slight dizziness and getting SOB easily on exertion. This tiredness started last year, and it is also associated with my eyes feeling tired and slightly dry, along with a burning sensation in my feet. I attributed this to be due to exam stress, and I did not think much of it. Since then, the burning sensation has gotten better, but the exhaustion has gotten worse. Small tasks get me so fatigued, and my mind feels cloudy. I am having trouble concentrating, and I got more forgetful and extremely absent-minded. I was so concerned about the fatigue and carried out an FBC, thyroid, and Vitamin-D level test. The tests for Vitamin D and thyroid were within the normal ranges. Something odd in my FBC was that my MCH and MCHC were low, but my hematocrit and hemoglobin were normal, just meeting the cut-off for ‘normal.’ What does this mean? Would I be considered anemic or not? I am taking Vitamin B12, Omega, and Magnesium supplements. Kindly give your opinion.

I can understand your concern. According to your statement, you have been suffering from exhaustion, fatigue, dizziness, shortness of breath, loss of concentration, forgetfulness, and absent-mindedness. I have gone through your investigation result and found that your MCH (mean corpuscular hemoglobin) and MCHC (mean corpuscular hemoglobin concentration) are slightly low than normal. You are suffering from the clinical features that may not result from slightly low MCH and MCHC. I think your typical clinical symptoms may result from anxiety disorders. You can get a good result by taking anxiolytic medications like SSRI (Selective serotonin reuptake inhibitors), Alprazolam, or Clonazepam.
