Hello doctor,

I performed oral sex on a guy. He did not ejaculate in my mouth, and neither did I have any cuts or sores in my mouth. I got tested six days after exposure, and it was negative. I started having symptoms at the beginning of next month, so I took a home test 36 days after exposure, which was negative. I ordered the fourth generation rapid test, which will be about 45 days after exposure when I receive it. Is there a chance that the test would be positive after negative on the other two tests? Kindly help.

I understand your concern. The chances of HIV (human immunodeficiency virus) transmission by single unprotected oral sex are almost nil unless there are bleeding wounds in the oral cavity. The chances of HIV tests coming positive later with given test results is nearly nil. He did not ejaculate in my mouth, but while performing oral sex on him, I vomited (gag reflex). Would vomiting cause cuts in the throat or mouth? What if the precum got in the mouth? Can the test at 45 days be conclusive?

Even if some precum gets in the mouth, it does not matter as long as there are no bleeding wounds in the oral cavity. In addition, vomiting does not cause cuts. Moreover, the initial HIV tests are negative. So the chances of HIV tests showing positive later is almost nil. Wait for the results. I would suggest getting an HIV antibodies test done in a laboratory after three months of exposure to be relieved and have conclusive results.

It is unlikely that the old missing tooth area would bleed at the time of the act. However, suppose he had bleeding gums, there are possibilities of transmission of HIV. The chances will be like infected blood or fluids in contact with the vagina. The risk is similar to unprotected intercourse, which is low with a single act. As HIV tests are negative, the chances of HIV tests coming positive later are almost nil.
