Hello doctor,

I am 24 years old. I have an external and internal hemorrhoid. I guess but I am not sure. I have a lump near my anus and it hurts a lot when I sit and walk. Sometimes it bleeds as well. There were two external hemorrhoids, but one was gone by itself and one remained and it has been there for more than 15 months and it is still there. I also noticed a color change near my butt area. I have attached a video. I want to know what actually it is and how can it be gone forever? Do I need surgery? (I do not want a surgery) Can it be cured by a home remedy?

Did you attach any video? Here only pictures are available (attachment removed to protect patient identity). Do you feel difficulty in passing stools? Any blood after stools? Does the area of lump itches or always seems to be wet? It does not seem to be like a typical external hemorrhoid. It needs a physical examination to confirm the diagnosis. Actually these are a collection of engorged small blood vessels and nearby soft tissue which occurs due to the pooling of blood due to prolonged constipation, straining, etc. Hemorrhoids can be avoided by encouraging more oral fluids, avoiding constipation, straining exercises especially abdomen, fiber-rich diet, etc. You most probably need to visit a general surgeon. Also, you might just need a minor surgical procedure. You no need to get worried. You can get discharged in two to three days from the hospital. I believe home remedies would not be helpful at this stage. Thank you and take care. Regards.

The video is 30MB in size, so I just uploaded a picture of it. I uploaded the video as well. Three or four days back I had very spicy food and since then I am really having a burning sensation after passing stools. It would last almost four to five hours. So I started drinking buttermilk and the burning sensation is very less and reducing by the day. I do not have any difficulty passing stools, it hurts only when I consume too much masala or chicken and the food which are hot for us (like chicken, green peas, too much masala in curries, etc). The area of lump itches when I walk too much or do any sort of exercises and yes it is always wet and smells too bad. The most irritating part is the smell, I cannot bear it and it bleeds only when I eat too much chicken. I am a bit worried about the color thing as well which is grey I guess. Is that normal? Also, how much does it cost for the surgery? Which hospital should I visit? Please help.

I have seen the video (attachment removed to protect patient identity). Yes, I can see the difference in color. You need to be a bit concerned regarding the color change and also the increase in foul smell. If it is external hemorrhoid, it could have been due to thrombosis of engorged veins (ischemic color change) and the foul smell might be due to an underlying infection. If you have started to feel much pain since the color change, it would be better to get evaluated and treated accordingly as soon as possible. No difficulty in passing stools, but discomfort on sitting, burning sensation after stools, itching, and wetness favors a diagnosis towards external hemorrhoid or a perianal soft tissue cyst with infection or even other rare diagnosis. If the mass is fleshy, it can be hemorrhoid. If the mass is fluctuant or boggy, it could be a cyst. It needs a doctor's physical access and evaluation to confirm any diagnosis as I said earlier. Please visit any recognized hospital in your local area. Ask for a consultation with a general surgeon. The surgery mostly would be minor either excision under local anesthesia or if its an infectious mass incision and drainage under GA (general anesthesia) might be required.
