Hello doctor,

I have increased head pressure when lying down, vision blackout when bending over, intense pressure when prone, and raising elbows. It is so bad I feel like I will pass out. My vision has gotten worse in the last year. This is more of a chronic issue. No headaches but some TMJ pain. I consulted an endocrinologist, cardiologist, and primary physician, and they set down those roads due to low BP.

Do you get a ringing sound in your ears (tinnitus)? What is vision loss like? Is it the same for both eyes? Is there a peripheral constriction of vision? Is there pain associated with vision loss? Is your body weight constant, or has it increased recently? Did you get your thyroid levels checked? What happened when you get up from a lying down position suddenly? I mean, do you feel giddy about getting up suddenly. How much are you stressed on a day-to-day basis? How is your mood on most of the days?

I get tinnitus about once or twice a day for a minute. My vision is blurry and out of focus but very random times, and it is the same for both eyes. And I do not have peripheral constriction of vision and pain associated with the vision loss. My bodyweight is constant. I got my thyroid levels checked, and all are under perfect optimal ranges. I get dizzy, and my vision goes dark, head pressure and racing heart while suddenly getting up from the bed. I have had pretty constant anxiety for a while and normal stress levels. I am anxious and down but for no real reason.

So gathering all of this, it seems you have mild orthostatic hypotension (OH). Orthostatic hypotension is confirmed by getting your blood pressure checked while lying down and then standing. If there is a fall of 20 mmHg systolic and 10 mmHg diastolic, it is confirmed as OH. Since we do not have that facility right now, we need to treat your symptoms.

Firstly, you should be well hydrated with water, juice, soups, etc. Secondly, you can start by wearing tight and long socks that extend above the knee. Physical maneuvers like not getting up at once and rather just taking a little more time to get up from lying down position may help. Since the vision problem is not static and dynamic, it should improve with the above measures.
