Hi doctor,

I have been getting anxiety or panic attacks. But this time, it is due to a friend of mine who passed away from a heart attack at the age of 36. I have been experiencing indigestion in my stomach area up to my throat.

What could it be? Is it anxiety related?

I have reviewed your history, stress related gastritis and peptic ulcers are common.

I would like to ask you some important questions.

1) What is the total duration of your symptoms?

2) Do you have unintentional weight loss?

3) Any family history of gut-related diseases or cancer?

4) Any history of blood in vomiting or stool?

5) Have you ever done any laboratory investigations?

6) Any stress-related symptoms worsening frequently or occasionally?

I would suggest some medications related to gastric acidity. Please consider capsule Pantoprazole 40 mg once daily half an hour before breakfast, syrup Gaviscon (Sodium Alginate and Sodium Bicarbonate) 10 ml after every meal and even at night if you feel acidity or burning sensation in your chest or abdomen. You have to consider some medications related to panic attacks or anxiety as well. This would improve your symptoms overall. And I am sorry to hear about your friend.

My symptoms started one week ago, and I fear of getting heart attacks. My chest and back hurt constantly, and there is indigestion as well. I feel my anxiety and panic attacks are in a constant loop.

I had a colonoscopy done, and this came back unremarkable.

I would recommend using acid suppression medicine as advised. It is great that your colonoscopy was normal recently. I think you are worried, considering an unfortunate incident around you.

Regarding the evaluation of your heart, you can have a simple ECG (electrocardiography) or exercise tolerance test along with an echocardiogram. These tests can tell whether there is any possibility of cardiac issues or not. But the important thing is to stay calm and avoid anxiety.

I called the ambulance twice, and an ECG was performed both times. Also, one of those times, I went to the hospital, and blood work alongside a chest X-ray was also done. In both of those times, everything came back unremarkable.

This is what I was assuming, considering your case history. If your cardiac evaluation is normal, stress-related gastritis and gastric ulcers are common, especially in stressful conditions like yours.

I would request you consider your anxiety evaluation. You may need some anti-anxiety medicine and stomach medicine which I have recommended.

Can you see my history with all the other doctors? My only concern is that the man who passed away apparently did a cardiac evaluation and his results were normal too.

And all I know is that he had been experiencing chest pain and breathing difficulties for two months. I am experiencing this also. I am quite active on the treadmill and weight training. I have no problem performing 45 minutes of cardio on a 10 % incline at 3.4 miles per hour.

I agree with your concerns, but during anxiety and stressful conditions like yours. I would suggest you visit a cardiologist then. He will be the person who can take a detailed history and perform cardiac examinations on you. He can write some advanced cardiac tests for you as well. I hope this will resolve your concerns related to your cardiac status.

I have an appointment with a cardiologist here. Basically, they have all said that it is not cardiac-related, and many have never diagnosed patients based on the symptoms I have described. On exertion, cardiac pain strikes. But I have it even when sitting down and resting (anxiety).

There are no stomach issues related to the heart directly in your ECGs (electrocardiography) and initial evaluation, which have already been performed.
