Hello doctor,

A few years back, I was convinced that I had MS. I had blurred vision in one eye and muscle spasms all over my body. I had a few MRIs which all came back lesion-free. I also had an EMG, which came back normal as well. Since then, I have also started having incontinence, and my arms and legs randomly go to sleep, and I get the tingly pins and needle sensation. I am worried that despite multiple reassurances that I have nothing wrong, I do have MS. Could this be possible that I would have had symptoms with no lesions? If not, could there be a reasonable cause for these symptoms?

I understand your concern and would explain that your symptoms are not suggestive of MS (multiple sclerosis), especially considering the normal repeated MRI. So, we can completely exclude MS in this situation. Coming to this point, I would recommend checking Vitamin B12 and Vitamin E plasma levels for possible deficiency, which can mimic this clinical situation.

Hello doctor,

I am also having generalized muscle fatigue as if I ran a marathon. I am noticing a Dowager's hump forming, and I am having pain in between my shoulders. The fasciculations still come. I have a very strong family history of back problems. Do you think it is possible that I have SMA 4?

It does not seem to be SMA 4 (spinal muscular atrophy type 4). I think that this could be just a wrong posture. I would also recommend checking Vitamin D plasma levels for possible deficiency. If all the above test results are normal, I would recommend considering fibromyalgia or chronic fatigue syndrome as the main cause underlying your complaints.
