Patient's Query

Hi doctor,

I frequently check whether I have locked the door when I leave the room, whether I have brought the keys with me, whether I have brought the purse in my bag, and whether I have put off the gas regulator. And all these make me anxious. Sometimes they give me anxiety or panic attacks. I doubt I have OCD, though I am not afraid of germs. I do not like the presence of dust on my hands or fingers, so I frequently wash my hands with water. I am scared of dust. And I use a little soap to wash my hands whenever I feel my hands are dirty, even with slight dust or dirt, because dust causes immediate allergic reactions on my skin. I noticed and experienced such symptoms 20 years ago at my primary school level. In addition, I find it difficult to do math calculations. I repeatedly do the maths calculation though I know the answer is correct. Also, my social life has not been good. All these recurrent symptoms make my life miserable. So I currently take tablet ES-Bliss 10 and tablet Etifree 0.5 mg for anxiety.

In addition, I experience other symptoms like, mood - I have anxiety, euphoria, hopelessness, loss of interest or pleasure in activities, mood swings, sadness, anger, guilt, general discontent, and apprehension. Also, I feel depressed, with feelings of grandiosity, agitated, impulsivity, restlessness, sudden increases in energy, irritability, and risk-taking behavior, and I show aggression sometimes. Cognitive - Sometimes, I believe that an ordinary event has special and personal meaning. But sometimes, I have delusions, thinking disorder, racing thoughts or false belief of superiority, frenzied speaking, doubting, and difficulty tolerating uncertainty. I expect things to be placed in an orderly and symmetrical manner. I experience frequent obsessive thoughts and compulsive behaviors, often causing anxiety, disgust, and unease. I often ask questions repeatedly to reassure.

I can understand your concern. You have OCD (obsessive-compulsive disorder) with depressive features. So I suggest you to follow few instructions to deal with your problem. Exercise for 30 to 45 minutes daily, like going for a walk. It will help uplift your mood. Meditate for 20 minutes daily; just sit in one place in a comfortable position and try to concentrate on your normal breathing. Do not stop or force yourself to stop your thoughts; just let them be. It will help you relax and reduce negative thoughts, anger outbursts, and impulsivity. Eat healthy home-cooked foods and avoid junk foods. Drink enough water. Spend quality time with friends and family daily or do video calls if you are living away from your family. Spend time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills, language, or musical instruments. Sleep for seven to eight hours daily. Avoid tobacco, alcohol, or any substance abuse. I suggest you to undergo a few laboratory tests like CBC (complete blood count) and thyroid profile, and also check for vitamin B12 and D3 levels. I suggest you to take capsule Fluoxetine (selective serotonin reuptake inhibitor antidepressant) 20 mg instead of Escitalopram (selective serotonin reuptake inhibitor antidepressant) to deal with your OCD with depression.
